High flow nasal cannula (HFNC) is een zuurstoftherapie voor hypoxemische patienten met milde tot matige respiratoire insufficientie.

Voordelen

• geen pijn of uitdroging neusslijmvlies
• geen echte contra-indicaties 
• patiënt ademt altijd het ingestelde zuurstofpercentage (FiO2) in
• geeft beetje PEEP (positief eind-expiratoire druk)

Resultaten sytematic review

• 8 RCT's met 1818 patiënten
• setting SEH en IC
• HFNC versus conventionele O2 therapie (neusbril, NRM of Non-invasieve beademing)
• Geen verschil in mortaliteit of intubaties
• beter patiëntcomfort
• HFNC versus neusbril of NRM: wel minder intubaties

Conclusie

• HFNC is een goed alternatief voor NIV en verminderd het aantal intubaties t.o.v. neusbril of NRM bij patiënten met hypoxemie
