Most people think getting in shape after 40 comes down to trying harder. It doesn't. I had chronic shoulder pain, chronic back pain, and joints that felt stiff every morning. I'm a plastic surgeon who sees this same pattern in patients every day. In the last 12 months I reversed all of it. At 47, I'm in the best shape I've been since my early 30s. It came down to three specific inputs I had been missing my entire career.
In this episode, I'm going to walk you through exactly what I eat and do every day, why protein and creatine are the non-negotiable foundation, how insulin control is the piece most people never address, and four steps you can start tonight.

âť“ QUESTIONS ANSWERED
How much protein do you actually need over 40 to change your body composition?Â
The target is 1 gram per pound of lean body mass daily. For most people this is significantly more than they are currently eating, and it is the single biggest lever for preserving muscle and changing body composition after 40.
Does creatine actually work for people who are not athletes or bodybuilders?Â
Yes. Creatine is one of the most-studied supplements in existence and supports both muscle energy production and cognitive function. After 40, neither muscle nor mental sharpness is maintained by default, and 5 to 10 grams daily directly protects both.
Why can't I lose fat even when I'm eating less and working out consistently?Â
When insulin is chronically elevated from a diet high in sugar and processed carbohydrates, your body stays in fat storage mode regardless of calorie intake. Controlling insulin is the hormonal prerequisite for fat loss. Calorie restriction alone cannot override it.
