A former air traffic controller says there are likely multiple factors to blame for a deadly plane collision in Japan.

12 Australians were among 379 people on board a commercial flight, which struck a coastguard aircraft at an airport in Tokyo.

Initial reports suggest the smaller plane, may not have been cleared to enter the runway.
