    Sleep surgeon Dr. Kasey Li returns to JawHacks three years after his first appearance for an in-depth update on nasomaxillary expansion and maxillomandibular advancement (MMA). Dr. Li developed EASE, a minimally invasive surgical split of the midpalatal suture used alongside an expander. He explains why he now uses the FME in about 95% of his EASE cases, the risks of overexpansion, how FME compares with custom MARPE, and why he sees no value in sleep endoscopy (DISE).   • #112 – Alex G. – Potentially an Insane Case of Protraction with MARPE

    Alex is a 22-year-old custom MARPE patient of Dr. Jeremy Manuele who started protracting three months after his split, wore a facemask up to 23 hours and 45 minutes a day, and pulled 5.5 to 6mm before going past the point where his bite still worked. He now has to jut his mandible forward to occlude, which makes him a live case study in why the mandible does not automatically follow the maxilla. Ron's usual asterisk applies: this is the frontier and nobody knows yet what adult protraction reliably does.
