Objectives:
At the conclusion of the MICIS learning session, the learner will have the ability to:
1. Appropriately recognize, diagnose and language opioid use disorder (OUD)
2. Compare pharmacologic treatments used in Medication Assisted Recovery (MAR)
3. Develop a strategy for treating acute pain for patients with OUD
4. Constantly consider harm reduction

Speaker: Elisabeth Fowlie Mock, MD, MPH, FAAFP 
Elisabeth is a Family Physician living in Holden, ME, and practicing part-time as an Adult Hospitalist (or “Nocturnist”) at Eastern Maine Medical Center in Bangor. She does consulting work and teaches for the Maine Medical Association’s Academic Detailing Program, MICIS. She attended Colby College and Emory University and received her MD from Vanderbilt University School of Medicine. After a residency in rural Family Medicine, she received her MPH in Health Policy and Administration from UNC-Chapel Hill. She is a Past President of the Maine Academy of Family Physicians and an Alternate Delegate to the American Academy of Family Physicians. Elisabeth very recently took her addiction medicine boards.

Dr. Fellers is a native Mainer. He completed his undergraduate studies in chemistry at MIT before attending Cornell University Medical College. During his psychiatry residency at the University of Virginia, he gained clinical research experience recruiting, running, and managing medication trials that use the human laboratory for the evaluation of medications for substance use disorders. Dr. Fellers further developed his clinical and research skills during an addiction Psychiatry fellowship at Oregon Health & Sciences University and a postdoctoral training fellowship at the Portland VA healthcare System.

Since his return to Maine in 2015, Dr. Fellers has led the development and implementation of several successful programs in response to the State’s opioid epidemic. The MMC IMAT Program provides addiction consultations for patients hospitalized due to serious medical complications of drug use, such as endocarditis and epidural abscesses. The MMP IMAT Program delivers Medication-Assisted Treatment with buprenorphine within all primary care practices. The MaineHealth/MBH IMAT Program builds on the success of these previous initiatives to deploy evidence-based addiction services across the entire MaineHealth footprint.

OBJECTIVES: 1. Discuss ways that hospitals can provide compassionate care for hospitalized patients with substance use disorder/opioid use disorder.2. Highlight challenges that inpatient units can face working with patients with substance use disorder/opioid use disorder.3. Discuss how systems of care can be better designed to support patients with SUD/OUD.4. Provide examples of how hospital staff and hospital units are developing best practice guidelines and training for staff around caring for patients with SUD/OUD.5. Discuss medical complications associated with injection drug use6. Discuss some of the interventions offered to assist patients with Opioid Use Disorder while inpatient7. Barriers to care through inpatient admission and at discharge

Parallels between Opioid Overdose and Suicide: Engaging the Conversation

1. Provide an overview of trends in suicide in Maine and parallels with the
opioid crisis and highlight potential relationships to substance abuse.
2. Highlight best practices on suicide identification, risk assessment,
intervention and management in healthcare settings.
3. Discuss systemic implementation of suicide prevention and protocol
development across hospital/practice systems.

Speaker: Daisy Goodman, CNM, DNP, MPH

Dr. Goodman will describe factors which complicate treatment of pregnant and parenting women with substance use disorders; identify key interventions to improve quality and outcomes and decrease healthcare costs for this population; and explore alternative models to help address barriers to care.

Dr. Goodman is a certified nurse-midwife and a clinical assistant professor of Obstetrics and Gynecology at the Geisel School of Medicine, and teaches quality improvement at The Dartmouth Institute. She coordinates women’s health services for the Dartmouth-Hitchcock Perinatal Addiction Treatment Program. Her research and work in quality improvement focuses on access to treatment and reproductive health care for pregnant and parenting women with substance use disorders, and the intersection between trauma and drug and alcohol use in women’s lives.

Objective: The objective and purpose is that attendees will have a greater understanding of opioid use disorder, including overdose prevention and recognition, use of naloxone, and laws pertaining to naloxone.

Speaker: Zoe Odlin-Platz
Zoe has been working in harm reduction since
2009. She is a Community Health Promotion Specialist with the City of Portland’s Public Health Division and oversees the daily operations of the Portland Needle Exchange Program. Zoe helped to implement both a community sharps box campaign and a naloxone distribution protocol. In 2012, Zoe
helped form I AM HERE Outreach Team, which is volunteer run and does street-based needle exchange, harm reduction, overdose response education and safe sharps disposal. This team is responsible for organizing the International overdose Awareness March and Vigil that is held each year in Portland. Zoe is a co-founder of the Maine Harm Reduction Alliance and chairs the education committee and sits on the Overdose Prevention Task
Force for the City of Portland, the Amistad Community Board and Dignity for Opiate Users. Currently Zoe and the needle exchange team are focusing on their opioid overdose reporting in and reversal data collection, agency trainings on harm reduction and overdose prevention and community naloxone distribution events.

Stephanie Nichols, Pharm.D., BCPS, BCPP
Stephanie is an Associate Professor of Pharmacy Practice at Husson School of Pharmacy, an Adjunct Clinical Assistant Professor of Psychiatry at Tufts School of Medicine, and a Psychiatric and Substance Use Disorder Pharmacist practicing at Maine Medical Center. She is highly engaged in interprofessional education of students, residents, and lifelong learners, both in the classroom and in the clinical setting. Her research has been published in: Pain, Current Psychiatry, Journal of Psychopharmacology, Journal of Studies on Alcohol and Drugs, Pharmacotherapy, Pharmacology, and Psychosomatics.
