Staff only reported 11 of 30 concerns to the Head over a period of thirteen years. The consequence of this failure - he was banned from teaching. Clearly the sanction didn't work. But what of the nineteen unreported concerns - the Serious Case Review overlooked this important issue. More than 30 children were abused.
