At EMS Today 2014 this past February, MedicCast host Jamie Davis, the Podmedic was joined in the podcast studio by John Riccio, MD, Medical Director of South Metro (Colo.) Fire Rescue Authority. He and Chief Rick Lewis developed the Advance Resource Medic (ARM) vehicle and deployment system, allowing for changes in dispatch and triage processes.

A mobile CLIA certified moderate complexity lab, used in conjunction with the skills and prescriptive authority of a physician extender, takes field capabilities and triage to a much higher level, allowing for a better patient experience, decreased costs and improved outcomes. Their service is using this approach to EMS care to lighten the load on local ERs and reduce call volumes while serving patients in place without a visit to the hospital.

MedicCast host Jamie Davis, the Podmedic is joined at EMS Today by Peter Dworsky, MPH, CBRM, NREMT-P, Corporate Director, and Scott Matin, MBA, NREMT-P, Vice President from MONOC (N.J.) Mobile Health Services. Dworsky and Matin conducted an original experiment and developed a powerful, educational video outlining the misconceptions and dangers of emergency siren use.

Jamie learns how they set up and conducted their unique experiment on ambulance siren use and also what changes were implemented in their EMS service following the outcome of the experiment. These types of experiments in emergency medical services will continue to challenge the preconceived notions we hold about age-old traditional operational procedures like how sirens and lights are perceived by others sharing the roads with us.

Scott and Peter later won an EMS 10 2015 Award for a video promoting hi-visibility wearables at roadside EMS scenes for safety.

FDNY paramedic Farooq Muhammad has innovated EMS care through the creative use of YouTube videos he creates on emergency medical services topics. The videos he creates promotes New York’s Fire Department EMS system and it also does a fantastic job of promoting high quality EMS care for the entire industry. Farooq is one of this year’s EMS 10 Innovator Awardees at EMS Today 2014.

Assistant Chief Edgar created a crew fatigue policy to objectively monitor crew activity and fatigue throughout their shift. The policy is designed to ensure the safety of the patient and crew, and transitions individuals from busy ambulances to reduce fatigue.

Their matrix system is designed to identify crews that may be fatigued and trade them out with fresh crews in order to combat medication and driver errors during patient care and transport. The city of West DesMoines looked at what flight crews do to address fatigue for flight safety and adapted those systems and programs to the EMS arena and their system specifically.

The primary focus was on crews slated to be available for long distance inter facility transfers that might take hours when the crew was already exhausted. Through both self-reporting and the matrix system, they were able to significantly reduce risk for medication errors and ambulance involved motor vehicle accidents.

Tip of the Week: Mangar Elk Provides EMS Lifting Assistance

This week on the tip of the week, I bring you the first of our segments for our product innovator picks from EMS Today last month in Washington, DC. Every year, our podcast team roves the exhibit hall floor, looking for interesting and innovative products that you would want to take a look at if you were there. Then we get together on the last day at the conference and pick the best of the best, the most innovative of the products we’ve seen. That’s what ends up in our finale episode from each conference.

This time around my pick was the Mangar Elk and Camel lif...

EMS 10 Innovator awardee Phil Callahan came up with a unique program that transitions returning US veterans to civilian EMS jobs as EMTs and paramedics using a specialized resilient educational program that manages the unique challenges the veterans are facing. The program doesn’t pathologize the educational problems of the veterans (doesn’t treat them as ill), it takes a straight educational approach to helping them overcome their issues with education.

The program has been wildly successful. The graduation rate of the students in the program went up considerably after the program was developed and now the veteran students are being retained at a higher rate than the other civilian students. This program and others like it are important parts of improving the lives of our returning veterans while providing highly trained and motivated paramedics to the work force here at home.

With so many systems moving to electronic health data collection and electronic patient care record (ePCR) tools, I thought it would be a good idea to bring in some expert advice on how to move in selecting and using an ePCR system. To do that, I asked Scott Streicher from SafetyPad.com to join me while we were both at EMS Today last month in Washington, DC.

Scott is the Chief Operating Officer at SafetyPad and his experience in helping EMS systems move to using ePCRs is a tremendous resource. We’ve had the folks from SafetyPad on the show in the past at other conferences, too, sharing their expertise.

Scott points out that the devices we can now use, like off-the-shelf Android tablets, make the transition to using an ePCR much more painless than you might think. A good software designer can utilize the built-in ease of use from these devices to make an intuitive patient care reporting system much like the paper or desktop computer systems you’re used to seeing.

Look for that ease of use when you are searching out an ePCR system if you’re part of the search committee or leadership team. Have some of your street providers try out the system and provide you feedback before you leap into a decision. Also, make sure the software designers are willing to work with you in a cost-effective manner to customize the system in some ways to meet your specific needs.

Victor Convertino from the U.S. Army research services in the U.S. Army Institute of Surgical Research. He has a background as a exercise physiologist and works on projects to find out more about how trauma and shock affect patient’s recovery and survival from injury. He is specifically working on managing patients with low blood pressure, hypotension, and low perfusion of tissues because of it.

The U.S. Army researchers like Victor are trying to give the U.S. Army medics the tools they need to keep the combat casualties alive until they can get to the higher echelon of care, the field hospital or beyond. Most casualties are lost on the battlefield before they get to the hospital facility so if we can identify tool and resources to use to limit and control hypotension and shock in the field, we can keep these patients alive until they get to the trauma center.

Victor and his team received an EMS 10 Innovator award for this groundbreaking research that will not only benefit the battlefield casualties but also the multi-system trauma patient in the civilian sector who might die before EMS can transport them to the trauma center otherwise.
