You are 100% right to audit every single chart note. Falsified records are real, and they can cost you years of your life and massive brain volume. Never, ever walk into a doctor's meeting alone—whether that means bringing another person to take notes, or recording the encounter yourself. If you are in a single-consent state, record it. It is your protection against being gaslit by a broken bureaucratic system.
Your primary job is not to be a 'good patient'; your primary job is to find a medical team that treats you like an equal partner. Stop begging for accommodations from doctors who don't hear you. Fire them instantly, and keep pushing until you find the provider who will give you 'fast-forward' treatments like Mavenclad. The future is brighter than you think, but you have to fight to get there."
