Joel and Ellie are trying to survive a post apocalyptic world, that has been taken over by a Cordyceps fungus that turns people into crazed creatures
Joel is a smuggler trying to get Ellie to a facility where they can test her as she seems to be immune to the infection.
Along the way they must fight not only the infected creatures, but other humans who do what they need to survive.

They have stated the Ellie will still be gay in the show they will not change anything about that
