We distinguish between simple and complex cases, noting that complex gastroschisis—defined by the presence of bowel atresia, volvulus, perforation, or necrosis—is associated with significantly higher rates of morbidity and mortality compared to simple cases. The discussion highlights the use of prenatal ultrasound markers, particularly intra-abdominal bowel dilation and polyhydramnios, to predict these complex outcomes and guide delivery planning. Finally, we review postnatal management strategies, including bowel protection, fluid resuscitation, and surgical closure techniques, emphasizing that while complex cases require longer hospital stays, the overall survival rate in high-resource settings exceeds 90%.
