Corrective Services Minister Paul Papalia has detailed a list of failures that lead to the self-harm and death of 16-year-old Casuarina inmate Cleveland Dodd.
Lack of body cameras, radios and staff attendance have all been mentioned as "significant failures" leading to the tragic incident.
