📌 Definition
• A neonatal condition where underdeveloped lungs lack surfactant, causing alveolar collapse and respiratory distress
• Also called Hyaline Membrane Disease
📌 Causes & Risk Factors
• Prematurity (<30–32 weeks)
• Male gender
• C-section without labour
• Maternal diabetes
• Genetic surfactant production issues
• Secondary surfactant deficiency (e.g. meconium aspiration, infection)
💡 Mnemonic: "Preemie Lungs Get MAD at Mum" – Prematurity, Lungs immature, Genetic, Maternal, Antenatal conditions, Delivery without labour
📌 Pathophysiology
• ↓ Surfactant → ↑ surface tension → alveolar collapse
• Leads to hypoxia, hypercapnia, acidosis → worsens surfactant → vicious cycle
• Right-to-left shunting in severe cases
📌 Symptoms
• Respiratory distress soon after birth
• Tachypnoea, grunting, retractions, nasal flaring, cyanosis, diminished breath sounds

📌 Differential Diagnosis
• Transient tachypnoea of the newborn (TTN)
• Pulmonary air leaks (e.g. pneumothorax)
• Infection/sepsis
• Meconium aspiration
• Congenital anomalies
• PPHN
💡 Mnemonic: "PAINT C-Me" – Pulmonary air leak, Aspiration, Infection, TTN, Congenital/Cardiac, Metabolic
📌 Diagnosis
• Blood gases – hypoxia, hypercapnia, acidosis
• CXR – ground-glass appearance, low lung volume, air bronchograms
• Lung ultrasound gaining popularity
• Echo to assess PDA or rule out CHD
• FBC, cultures, glucose, electrolytes
📌 Management
• Early CPAP for milder cases
• Surfactant replacement – selectively after stabilisation
• Advanced: intubation/IPPV if severe
• Supportive care – fluids, glucose, thermal regulation, antibiotics, nutrition
💡 Emerging technique: MISA – minimally invasive surfactant administration while on CPAP
📌 Complications
• Pneumothorax, PIE
• PDA
• Pulmonary haemorrhage
• NEC, sepsis
• Intraventricular haemorrhage
• Chronic lung disease (BPD)
• Retinopathy of prematurity (ROP)
• Neurodevelopmental delays
📌 Prognosis
• ⭐ Excellent with early steroids & surfactant
• Mortality <10% in high-income settings
• Long-term issues more likely with extreme prematurity
