Endocrine therapy is currently the cornerstone of treatment for advanced hormone receptor–positive (HR+) breast cancer in both pre- and postmenopausal patients. However, not all advanced HR+ breast cancers respond to first-line endocrine therapy, and those that do respond eventually relapse. Agents that target critical pathways involved in resistance to endocrine therapy, such as cyclin-dependent kinase (CDK) 4/6 inhibitors, have been approved by the US Food & Drug Administration (FDA) for the treatment of HR+/human epidermal growth factor receptor 2–negative (HER2−) advanced/metastatic breast cancer. Now, CDK 4/6 inhibitors are being investigated for the treatment of early stage HR+, HER2- breast cancer.
This web-based, on-demand, activity will feature interprofessional perspectives and expert insights on the latest emerging evidence for CDK 4/6 inhibitors in the adjuvant setting for early breast cancer. The faculty panel will also provide their perspectives and best practice recommendations for improving adherence and side effect management. Case-based discussion will provide practical approaches for integrating CDK 4/6 inhibitors into real-world clinical practice for the treatment of early stage HR+, HER2- breast cancer.

In this case-based activity, Drs. George Bakris and Biff Palmer discuss the benefits of RAAS inhibitors in the patient diagnosed with diabetes, hypertension, and chronic kidney disease. Cholesterol guidelines recommend nonstatin, LDL-C-lowering therapies in very high risk ASCVD patients if LDL-C goals are not achieved (ESC/EAS dyslipidemia guideline) or if LDL-C is above a specific threshold of 70 mg/dL (AHA/ACC cholesterol guideline). Unfortunately, a substantial number of patients with ASCVD do not achieve LDL-C targets, and nonstatin, LDL-C-lowering therapies are underutilized. Therefore, patients such as those who have experienced an acute coronary syndrome (ACS) remain at increased risk for another major cardiovascular event. Patients with a recent ACS are especially vulnerable to a recurrent cardiovascular event. Nonstatin, LDL-C-lowering therapy with PCSK9 inhibitors and ezetimibe has demonstrated reductions in cardiovascular risk after an ACS. PCSK9 inhibitors have demonstrated robust LDL-C lowering to very low LDL-C levels, with no increase in adverse safety events and with additional cardiovascular risk reduction. Analyses of cardiovascular outcome trial data with PCSK9 inhibition have indicated particularly potent cardiovascular risk reduction in patients with a recent ACS; utilizing PCSK9 inhibitors in these patients enables clinicians to greatly improve their patients’ clinical outcomes. Experts in the immunopathology of type 1 diabetes review data showing that clinical presentation is preceded by an asymptomatic phase, characterized by the presence of one or more islet autoantibodies. Screening children for islet autoantibodies during the two preclinical stages can identify those with multiple autoantibodies and high-risk antibody phenotypes who may be candidates for preventive interventions before developing dysglycemia or the clinical onset of type 1 diabetes.
