In our modern western culture, we don't understand the concept of a betrothal. The closest thing we have is an engagement, but even in the engagement, we don't see the same level of commitment as in a betrothal. In the engagement, there is still the option to back out of the marriage, but in a betrothal, the commitment has already been made; the marriage has been all but consummated.
