The Radiation Machine That Changed Software Safety Forever

A cancer patient lies alone inside a radiation treatment room. Outside, an operator enters a prescription into a computer, checks the screen and starts the machine. The console says that almost no radiation has been delivered. The patient, however, has just received an enormous overdose.

Between June 1985 and January 1987, six known patients were massively overdosed while being treated by the Therac-25 medical linear accelerator. Several suffered devastating injuries and some died.

The accidents became one of the defining warnings in the history of software engineering because the computer was not merely keeping records or displaying information: it was controlling a machine capable of delivering potentially lethal radiation.

The story is often reduced to a notorious software bug. That is true, but incomplete.

The deeper failure was that software defects were allowed to become physically catastrophic because independent hardware protections had been removed, operators received misleading information, safety analysis largely discounted software failure, earlier warning signs were not connected quickly enough and the manufacturer initially struggled to accept that its machine could be responsible.

That is also why Therac-25 still matters. Modern medical devices, cars, aircraft, industrial equipment and AI-enabled systems increasingly place software between a human decision and a physical consequence.

Therac-25 showed what can happen when the software becomes part of the safety system but is still treated as if it were merely another convenience.

How A New Aircraft Became A Global Crisis

Two new Boeing passenger aircraft crashed less than five months apart. All 346 people aboard them died. At the centre of both disasters was a flight-control system most passengers had never heard of and many pilots had received little or no training about: MCAS.

But reducing the Boeing 737 MAX disaster to a software malfunction misses what made the episode so important.

Investigations eventually exposed a deeper chain involving aircraft design, assumptions about how quickly pilots would respond to emergencies, Boeing's determination to preserve commonality with older 737s, weaknesses in regulatory oversight and crucial information that did not reach the pilots expected to control the aircraft.

The story still matters. The MAX returned to commercial service after extensive modifications, but scrutiny of Boeing's engineering, manufacturing and safety culture continued long afterwards.

As recently as July 2026, the Federal Aviation Administration was still describing enhanced inspections and oversight as it cautiously restored some certification authority that Boeing had lost following the MAX crisis.

The Nuclear Protest That Triggered A Secret Operation

Shortly before midnight on 10 July 1985, two explosions tore through a Greenpeace ship moored in Auckland Harbour. The Rainbow Warrior sank at the wharf, photographer Fernando Pereira was killed, and what initially appeared to be an extraordinary act of sabotage soon became something far more serious: an operation carried out by the intelligence service of one of New Zealand's supposed friends.

France had sent agents of its external intelligence service, the DGSE, to New Zealand to stop the Rainbow Warrior sailing towards the French nuclear test site at Moruroa.

The operation involved surveillance, false identities, forged passports, specialist divers, explosives and several separate teams.

Four decades later, the affair remains one of the clearest examples of a democratic state using covert action against a peaceful civilian organisation on the territory of another democracy.

What Is Really Happening At America’s Most Mysterious Ranch?

For more than three decades, a remote cattle ranch in northeastern Utah has been associated with claims of unidentified flying objects, mutilated livestock, electromagnetic disturbances, strange creatures, radiation spikes and equipment apparently behaving in ways its operators struggle to explain.

What separates Skinwalker Ranch from countless paranormal stories is not simply the strangeness of those claims. It is the extraordinary amount of money, technology and institutional attention that has subsequently been thrown at trying to investigate them.

The ranch is still being actively investigated in 2026, with rockets, drones, LiDAR scanners, GPS equipment, electromagnetic sensors, cameras and increasingly elaborate experiments being deployed around several supposed hotspots.

Yet the central paradox has barely changed: researchers repeatedly report anomalies, but no publicly available experiment has yet established a repeatable phenomenon requiring anything paranormal, extraterrestrial or beyond known physics.

That makes Skinwalker Ranch more interesting, not less. There is enough documented history to reject the idea that the entire story was simply invented for television, but nowhere near enough independently replicated evidence to conclude that aliens, portals or interdimensional beings are responsible.

So what actually happened there — and which pieces of evidence deserve to be taken seriously?

What Happens If Machines Become Too Powerful?

Artificial intelligence does not need to become conscious, angry or evil to become dangerous.

The more serious fear is much simpler: humanity could eventually build systems that are extraordinarily capable, give them increasingly important objectives and access to the real world, then discover that controlling exactly how those objectives are pursued is harder than expected.

That possibility is no longer confined to science fiction. The 2026 International AI Safety Report concluded that today's systems still lack the combination of capabilities required for a genuine loss of human control, but researchers are seeing progress in autonomous planning, situational awareness, deception, cyber capability and other behaviours that could become relevant if future systems grow substantially more powerful.

The existential-risk argument therefore is not that an AI apocalypse is happening now. It is that waiting until the danger is obvious could leave humanity solving the hardest safety problem it has ever faced after the technology has already arrived.

The Black Pad Mystery

On Saturday, December fourteenth, nineteen ninety-one, the distance separating two homes in Coventry is almost absurdly small. Eighteen-year-old Nicola Payne is at her boyfriend Jason Cooke’s address in Winston Avenue. Her parents, John and Marilyn Payne, live nearby in Woodway Close.

Depending on the route and the later account being used, the walk between them is described as roughly five, seven, eight or ten minutes. Either way, it is not a journey that should require planning.

Nicola has a seven-month-old son, Owen. She and Jason are preparing to move into a three-bedroom house, with Christmas approaching. The trip to her parents’ home is familiar territory through a part of Wood End she knows well. One possible shortcut crosses an area of waste ground locally known as the Black Pad.

At around lunchtime, Nicola leaves Winston Avenue on foot. Nothing publicly established about the decision suggests anything remarkable. She is simply making a short journey between two addresses connected to her family.

Enough time passes for that ordinary assumption to stop working.

Nicola has not reached Woodway Close.

And a journey measured in minutes is about to generate questions that will still have no complete answer more than three decades later.

Every Major Clue Examined

On Second Street in Fall River, Massachusetts, the Borden household runs according to routines that are familiar to everyone inside it.

Andrew Borden is a wealthy businessman with a reputation for thrift. His second wife, Abby, manages the home. His adult daughter Lizzie lives there with them, as does the family’s domestic servant, Bridget Sullivan.

The house itself is unusually important. Rooms connect awkwardly. Doors are routinely locked. Andrew is sufficiently security-conscious that the front entrance has multiple locks. Anyone moving through the building has limited options for avoiding the people already inside.

On the morning of August fourth, eighteen ninety-two, nothing initially announces that this ordinary domestic geography is about to demand extraordinary scrutiny. Andrew leaves on business.

Abby works around the house. Bridget cleans windows. Lizzie remains at home. Lizzie’s older sister Emma is away, while the sisters’ maternal uncle, John Morse, has spent the previous night in the house but leaves that morning.

Within hours, investigators will have to reconstruct precisely where everyone went, which doors were locked, when individual members of the household were last seen and whether a stranger could have entered without being noticed.

Their problem is that the answer to almost every important question will remain frustratingly incomplete.

How A Beautiful African Lake Became A Silent Killer

On the evening of 21 August 1986, something almost unimaginable happened in a remote part of north-western Cameroon.

There was no huge volcanic explosion, no advancing wall of fire and no violent earthquake announcing what was coming. Instead, an enormous quantity of invisible carbon dioxide escaped from the depths of Lake Nyos and began flowing silently into the surrounding valleys.

By the time it was over, at least 1,700 people were dead. Thousands of cattle and other animals had also been killed, while entire settlements had been devastated. Many victims apparently had almost no opportunity to understand what was happening before they lost consciousness.

The catastrophe became known as the Lake Nyos disaster, and it exposed a natural hazard that few people outside specialist scientific circles had even considered possible. A lake had effectively stored an enormous quantity of gas beneath its surface and then released it catastrophically into the atmosphere.

Even four decades later, Lake Nyos remains one of the strangest natural disasters ever documented. It was a disaster in which the water itself did not sweep through villages.

The volcano beneath the landscape did not erupt in the conventional sense. The real killer was something nobody could see.

Inside One Of History’s Strangest Nuclear Accidents

In September 1987, two men entered an abandoned medical clinic in Goiânia, Brazil, looking for equipment they could sell as scrap. What they eventually carried away in a wheelbarrow looked like valuable machinery. Hidden inside it was a highly radioactive source containing cesium-137.

Within days, the source had been broken open. A strange blue glow appeared inside the capsule. People gathered around it, touched the material, carried fragments home and even rubbed the radioactive powder onto their skin because they thought it was beautiful. By the time anyone understood what was happening, radioactive contamination had travelled through houses, scrapyards, public transport and neighbourhoods across Goiânia.

The phrase that the powder "poisoned an entire city" should not be taken literally: most of Goiânia's population was never significantly contaminated. But the accident became a city-wide emergency. More than 112,000 people were eventually screened, hundreds required decontamination, 249 were identified as significantly contaminated and four people died directly from the radiation exposure.

Almost four decades later, Goiânia remains one of the most important radiological accidents ever studied. It was not caused by a reactor explosion, a nuclear weapon or a secret military programme. It began with something far more mundane: a dangerous medical source was left where ordinary people could reach it.

Is Reality Real?

Everything you have ever experienced could, in principle, be happening inside a simulation. Your memories, your body, the Earth beneath you, the stars above it and perhaps the entire observable universe could be information generated by something existing at a deeper level of reality.

There is no scientific evidence proving that this is true.

Yet the simulation hypothesis has survived its transformation from science-fiction premise into a serious philosophical problem because one argument makes it surprisingly difficult to dismiss.

If advanced civilisations could eventually create enormous numbers of conscious simulated people, why should we assume that we happen to be among the relatively small number of people living in the original reality?
