Welcome back, everyone, to another exciting episode of TBSA!
Today, Luke and Jake dive into topical antimicrobials. Obviously, this is a huge subject in burn care. In order to complete a thorough discussion of all of the various agents, best case use arguments, and the pros and cons of each, we are going to need to break this one up. So, for today, we just want to highlight the choice of topical antibiotics that are typically used in outpatient burn care. This episode will hopefully prepare providers to go down to the ER, see a new patient, and optimize them for outpatient wound management. Take a listen!
As always, if there are topics you would like us to discuss, we want to hear from you! Reach out to us on social media or on email. We love to here from our peers as we continue to discuss our passion: burn care!

Welcome back, everyone, to another exciting episode of TBSA!
Today, Luke and Jake discuss exfoliating skin conditions. Specifically, we are highlighting Stevens-JohnsonSyndrome and Toxic Epidermal Necrolysis. While these diseases are not due to thermal injuries, they are—inherently—one of the pathologies that cause skin failure.
Biomarkers, treatments, pathology findings, and a whole host of management decisions come into play with this. When do we transfer? Who gets transferred? What subspecialty expertise do you need to round out the multidisciplinary approach to managing these patients? All of this (and more!) will be covered in this episode. Take a listen!

TBSA is back in the studio, and we are coming at you with a real riveting episode on all the in's and out's of central access.
Jake and Luke cover it all in this one: biases they have, the data behind how to manage these devices, and some interesting findings on bundle optimization. As we frequently say, perfect isn't on the menu, and nothing is free. Similarly, central lines are not without inherent risk to the burn patient. Take a listen and see if any of your presumptions are aired out on this episode!

Welcome back, everyone, to another exciting episode of TBSA!
Today, Luke and Jake dive into burn sepsis. Always a humbling aspect of burn care, it is so critical to give the right antibiotics to the right patient at the right time. Unfortunately, typical biomarkers and physiologic parameters in typical septic ICU patients get thrown out in burn care. Listen to Jake and Luke as they walk through the issues with burn sepsis and how we approach it.

You can feel the chemistry in this episode between Jake, Luke, and the subject at hand: chemical burns!
Jake and Luke review some basic chemistry facts to explain why different chemicals behave in such varied ways in the human body. Further, we go over specific toxidromes associated with these agents. Lastly, we review ourapproach to these patients and specific, can’t miss elements to successfully treating these patients.

Welcome back, everyone, to another exciting episode of TBSA!
This week we revisit a favorite topic: CRRT. Specifically, high volume CRRT. This modality of CRRT leverages using high circuit speed to pull large volumes of blood in an effort to clear immunoglobins and infectious material from the blood. At least, that's the theory! Take a listen as Luke and Jake cover the exciting research on this emerging topic.

Now that we have completed our walkthrough on the first 48 hours of the BICU stay, tag along with Luke and Jake as they navigate the hardest part of the ICU stay: discussing the goals of care with the patient's next of kin.
We talk about how important it is to pass on a holistic view of what the patient's hospitalization will look like. Further, we discuss the necessity of involving the multidisciplinary care team early and often in order to ensure a cohesive message is presented to the patient's family.

Welcome back, everyone, to another exciting episode of TBSA!
Today, Luke and Jake unpack another hot topic (please forgive the pun): DVT prophylaxis! With the everchanging pharmacokinetics and pharmacodynamics of burn patients, prophylaxis dosing can often feel like a moving target. Join us as we unpack the right drug, dose, monitoring, and timing for starting DVT prophylaxis. This issue is quite unique in burn patients.

Our next episode dropped! Follow along with Luke and Jake as we take a deep dive on a fundamental--and inherently difficult--aspect of burn care: burn hypermetabolism.
Getting this right is difficult, and getting it wrong has significant downstream morbidity and mortality risks.

Electrical injuries can be intimidating to many providers. These burns--more than many others--are associated with significant whole body morbidities that can be far removed from the thermal injured tissues themselves.
Luke and Jake walk through the major management considerations for these injuries. Cardiac manifestations, ocular injuries, and compartment syndromes are covered. Take a listen!
