The Circle Of Insight

Why can a compliment make someone feel wonderful for an hour, only for the insecurity to return almost immediately? The constant search for reassurance may look like vanity or attention-seeking, but self psychology suggests something deeper may be happening. Heinz Kohut’s work helps explain how validation from others can sometimes become psychological scaffolding for a fragile sense of self.

You promised yourself you would never sound like your mother or father—until one day their words came out of your mouth. Psychology suggests that our earliest relationships create internal templates that can quietly influence how we love, argue, parent, and respond to rejection decades later. This episode explores how object relations, identification, and repetition can turn childhood experiences into adult patterns—and why recognizing them can give us more choice.

Heinz Kohut broke from classical psychoanalysis to argue that the self isn't shaped by drives and conflict alone, but by whether early relationships mirror, idealize, and validate who we are. This episode unpacks Kohut's self psychology — mirroring, idealizing, and twinship needs — and how empathic failures in those early bonds can leave the self fragile or defensively grandiose later in life. We trace how this framework reshaped clinical thinking on narcissism, moving it from a fixed diagnosis toward a developmental injury that can be understood and worked with in treatment. It's a concise but substantive primer on one of psychodynamic theory's most influential departures from Freud.

This review looks at a recent summary of about a dozen clinical trials that claimed omega-3 supplements improve irritability and communication in autistic individuals.
It compares the actual findings which showed small, often statistically meaningless results based on subjective questionnaires rather than real-world changes with the much bolder headlines seen by the public. While the idea makes sense in theory, the underlying studies suffer from small sample sizes, loose research methods, and a tendency for benefits to fade under closer scrutiny. Ultimately, the evidence isn't strong enough to call omega-3 supplements a dependable treatment.

While biomarkers offer significant clinical promise by enabling earlier diagnoses, personalized therapies, and objective disease monitoring, they often fail to correlate with meaningful patient-centered outcomes such as quality of life and long-term survival. This disconnect is particularly stark in psychology and psychiatry, where complex conditions lack definitive biological signatures and risk being over-medicalized by surrogate metrics. Furthermore, practical challenges—including high costs, accessibility disparities, and the risk of false positives leading to unnecessary interventions—demand substantial caution. Ultimately, the script argues that biomarkers should be treated as fallible diagnostic adjuncts guided by intellectual humility and comprehensive clinical judgment rather than definitive stand-ins for patient health.

And this episode will give you a brief look at a case conceptualization I did of George Costanza and we look at it through the lens of the psychodynamic diagnostic manual number three the PDM three so we’ll look at categories like reality, testing personality, organization, defense, mechanisms, organizing theme, relationships, developmental, formulation, and more

This episode breaks down the difference between genetics and epigenetics — the genes you're born with versus the environmental and experiential factors that switch those genes on or off. The conversation explores how stress, trauma, and lifestyle can alter gene expression without changing the underlying DNA sequence itself, and what that means for understanding mental health and behavior. It's a foundational look at why "nature vs. nurture" is really a false choice, and how the two are constantly interacting.

Carlos talks with Dr. David Grand, the developer of Brainspotting, about how specific eye positions can access and help process unresolved trauma stored in the brain and body. The conversation covers the theory behind the technique, how it differs from more traditional talk therapy approaches, and why it's gained traction among clinicians treating PTSD and deep-seated trauma. It's an inside look at a therapy built on the idea that "where you look affects how you feel."

this conceptualization assesses Lorelai Gilmore’s neurotic personality organization, highlighting how her trademark charm, rapid-fire humor, and intellectualization serve as higher-level defenses against emotional vulnerability. The formulation centers on her core conflict between an intense longing for connection and a pervasive fear of losing autonomy, rooted in early developmental experiences within a rigid, conditional family environment. Ultimately, it outlines the clinical implications for therapy, emphasizing the need to navigate her avoidant transference dynamics and maintain therapeutic boundaries without colluding with her charismatic deflections.
