What do you want to have happen when extended care is needed?  Where do you want to live?  It's much easier to have these discussions today, while you're not in the middle of a crisis.

Have the conversations now so your family and loved ones will have a blueprint of what you have and what you want.  They'll then be able to make better decisions for you and with you regarding your care.
