Raise your hand if efficiency has always been one of your favourite words. If it's up, this one is for you.

Efficiency is what got you here. It's why you got into medical school, matched into residency and built a practice that works, mostly. But there's a version of you that has out-efficiencied yourself, and you might already be living in it.

Here's the problem with efficiency: it doesn't shrink your workload, it multiplies it.

Being efficient doesn't mean less to do. It means you keep saying yes, because you can do it well, and it feels easier and faster to just do it yourself. But every yes becomes one more thing you're now doing in less time than the last.

Picture this: it's 3am and you're running through the charting that's still left to finishs, the supply order still waiting on you, the paperwork that's still yours to finish. That's not an efficiency problem. That's a sprint you've been running since residency, and a marathon (aka your career) doesn't work that way.

"The goal isn't just to be more efficient. The goal is to figure out what you actually need to do."

What you stop doing matters more than how fast you do it.

Once efficiency stops being the goal, the question changes. Not "how can I do this faster", but "does this need me at all". That single question, deciding what doesn't need your expertise, can be worth 8+ hours back a week.

When you're doing what actually needs you to do it, your efficiency superpower gets to work on what needs it.

You're wrapping up a visit, already halfway out, when you see it: a bag opening, an envelope sliding out. You know before the patient says a word. Your energy plummets. More insurance paperwork. Another third party form to add to an already never ending task list.

Then the mental math starts. Are you already behind? If you do this now, does the whole afternoon get further derailed? All while outwardly trying to keep your face neutral so the patient doesn't know your internal battle.

"It has to get done, but when is it going to get done?"

This interrupts your flow, causes you stress and takes more energy to figure out how the math will math.

You already know before they open their mouth

That moment repeats itself...a lot. The paperwork itself often doesn't take that long. But compound that by 3-5 patients with forms per clinic...and that's 15 more minutes. Because it doesn't have a fixed place to go, so it gets squeezed into clinic, or pushed to evenings, or weekends.

Time block your third party paperwork: this is where it gets done

The obvious block of time can look like tacking it at the end of the afternoon. But end of day time is never reliable. A late patient, a call to return, you're also just tired and the block gets bumped. And then you're right back to where you started.

Find a time that (reliably) works for you and for your life

What works is one consistent block of time, protected the same way you'd protect a procedure day. Once your team knows when it happens, they can tell a patient when their form comes back before you're ever asked. The clinic room envelope moment doesn't happen as often.

Giving the third party paperwork a home: less interruptions and stress for you. Your team doesn't feel bad when they ask you about it...again. Patients know what to expect.

What question do you answer every week... repeat?

You know the one. Mid note, mid patient, someone leans in with something you have already answered before. A patient showed up late, can she still be seen? Should this add on go here or there? What do we tell him about his work note? None of it is hard. It "only" takes a few seconds. All of it adds up.

Answer before the question gets asked

We know these questions are coming (because they always do). Which means we can answer them before they get asked by giving your team the answer and the permission to answer it without you. Write down the decision, the one already running in your head, and hand it over. Not a policy. A sentence.

The math on a few less interruptions

A few interruptions a day is 15 -20 less/week. Cut those down and the difference is not subtle. Less time explaining, less energy spent trying to control your face while internally containing frustration, more of your note finished before the next patient walks in.

You are giving them the permission to do this without you because this is how you want it done and it's written down.

Everybody wins when they know how you'd answer and they feel confident to make the call without involving youYour team does not want to interrupt you either. Give them the rule or guideline and their job gets easier, which means yours does too.

You know the one. The question you answer so often you can guess it's coming before the patient finishes asking. It feels small. It is not free.

Why That Question Costs More Than a Few Minutes

It interrupts your flow, adds a task to your EMR list and usually means the patient carried worry before they even called. It adds up, three minutes here, a phone call there, the dread of opening the task list...again.
