In an exclusive interview with TheRightDoctors on Emcure AICOG tv, Dr. Manish Pandya, professor at Head of The Department of Scientific Research Institute from Surendra Nagar, Gujarat spoke about anemia and its cure in India. This is what he said :

' Anemia is defined by W.H.O as less than 10 gm percentage of hemoglobin in postpartum period. It affects health of both mother as well baby and it has got lots of complications.

A lady who is talking 1 tablet of any iron preparation, hemoglobin increase will be seen after one month so you cannot prescribe oral preparation in a lady who is moderately or severely anemic.

For a normal routine pregnancy you can give oral tablet, but when you are talking severe anemia or moderate anemia at that time injectable formulations are available in market in form of iron sucrose.

We were using iron sucrose. When I was in medical college as a professor in HOD. We have done study of 500 postpartum patients. Every postpartum lady must have to take 2 injections of iron sucrose once she get discharged and we found that there is increase in hemoglobin

Second most important thing, what alternative you can give instead of FCM that is the blood transfusion we are giving a disease free increment into the hemoglobin of a lady by giving the simple Ferric carboxymaltose within 50 minutes and patient is behaving better

'Cervical cancer screening is very important and are becoming major of maternal mortality these years.

We have two methods of preventing cervical cancer. One is by vaccination and second is by the screening.

Vaccination should start at the age 9 years to 15 years before the girl child had exposure to sexual contact and screening then comes later on because cervical has a long precancerous spores where we can screen these women.

Indian pediatric association has taken up cervical cancer screening program, but the government of India is not able to afford it because of the high cost, so at some places like in Punjab and other places they have acquired some vaccines.

FOGSI is doing lots of work on cervical cancer screening. I had taken it as my main agenda during my vice presidential year 2012-2013 in FOGSI'

' As far as the Indian scenario is concerned regarding the maternal mortality due to PPH is not that good.

Everybody has to be sensitized that postpartum hemorrhage should not be a major killer in India and we should build our patients by improving their hemoglobin level by carrying out safe delivery at the institutional level

FOGSI is doing a good work now to control the PPH and to save our mothers. They have a main agenda to save mothers and save uterus.

There should be more sensitized workshops. In medical college as well as in all societies or organization that it is equally a surgical emergency so we gynecologist now very well qualified surgeons, now we are MS surgeon so we easily go for a surgery if needed.

Every PG students and every gynecologist should go for a PPH tackling surgeries on a demo that is on uterine models .

Then you can tackle the postpartum hemorrhage and as now-a-days the rate of cesarean section is on rise so when the uterus is in your hand, when the abdomen is open, the gynecologist can easily spear few minutes to control the postpartum hemorrhage.'

Everyone should finish their family before the age of 30 years. Late marriage has become the norm in this 21st century.

If the wife does not get pregnant within one year of marriage and she is less than 30 years of age or she is more than 30 years and married for 6 months she should consult a fertility specialist.

If you are 30 year by default the egg quality & quantities start decreasing.

Stress you can reduce, by doing yoga, by doing meditation, you have to abstain from addictions like tobacco, alcohol, smoking.

Do life style modification, remember the fertile period, have balance diet, abstain from addictions, see if you get pregnant within 6 months of time if you do not, please visit a fertility specialist.'

Women enter into pregnancy in such a poor state of health and that worsens during pregnancy Because of lack of education number #1, lack of nutritional support and the system factor, which needs correct this, needs proper timely interventions

These lead to lot of complications and that causes the high maternal mortality in India.

the seeking behavior, pre-care seeking behavior and when they come to the hospital the personalize lack of care in the institutes plus a proper intervention,

Counseling, is less commonly observed in many of the institutes and support further women from the family what treatment is advised whether it is adhered so compliance also matters so when these factors are adversely affecting in India

That is why we are having high maternal mortality and morbidity.'

' clinics are transferring 3 embryos, 4 embryos and 5 embryos in their patients and the major problem. When you transfer so many embryos is that there is a high risk of having multiple pregnancies It is a very high risk in IVF.

Once the patient has actually sold their houses or given them for lease, if you give them twins or triplets later on they do not have the money to deal with those pregnancies. Single embryo transfer is the need of the hour

IVF is rapidly advancing, not many centers have a proper NICU and when you have multiple pregnancies these women have a higher chance of having PIH, higher chance of having eclampsia, higher chance of having diabetes, hypertension.

Twins have higher risk of growth retardations. Pediatricians compared singletons and multiples and found out that there was huge developmental delay as far as growth is concerned in twins or more than that.'

"Women play the most important in a preventive role not only the family, but also in the health of the nation so looking after her health empowering her, respecting her is the only way to take her and the nation to the top.

The first program, is Adbhut Matritva. Now this program is all about making motherhood of a very unique and incredible experience for the patients. this is a very unique module which start with preconception counselling then in the antenatal period

It looks after all the aspects of pregnancy starting from first trimester what should be done, what should you expect, how many visits you should actually you know dedicate towards your antenatal period and how many times you should to the doctor, what immunization is required , What kind of diet in the first trimester, then modification in the second trimester and third trimesterand what exercises has to be done, little bit of yoga and meditation which is very important

We did a survey and we found out that only about 30% of the doctors basically, early initiation of breastfeeding is there and that is very important aspect, so we have join hands with the breastfeeding promotion network of India. We have joined with the PSI. PSI will take care of the contraception post-delivery

The neuropsychology of the child is important. Why a certain child behaves in a certain manner? All is decided in utero. So women must be as calm as possible, look after your health as much as possible, eat the right things, you know take the immunization on time, go to the doctor on time.

Second important program, which we are taken up called Manyata. Manyata is about accreditation of the hospitals. Most of our hospitals are not accredited and once they are not accredited the right kind of evidence based practices are not there. "

We have got different tissues like Mesonephric tissue or Wolffian tissue in male.

The problem of un differentiation or mal development creates all the problem meaning that the birth passage or the vagina is not developed or partly developed.

We examine the girl clinically and take the history, sonography and there are other investigations including the magnetic resonance imaging. We come to know which area is defected where is mal agenesis or maldevelopment

If the uterus is partially developed then we can construct it, if the uterus has got some mal development we can unify, if there is septum or partition between the two, we can remove the partition.'
