Q: Is TRT dangerous for your heart?
A: The concern came from a 2001 study called the TOM Trial, which was halted early and studied men in their 70s and 80s with preexisting cardiovascular conditions. The TRAVERSE Trial, published in 2023 and enrolling over 5,000 men with symptomatic low testosterone, found no increased cardiovascular risk. The field moved. The warning did not.
Q: Does TRT cause permanent infertility?
A: TRT suppresses natural testosterone production and reduces sperm counts, but this is manageable and not permanent. Adding HCG or clomiphene to the protocol preserves testicular function and restores sperm production when needed. For men who want to protect fertility, there is a protocol, and it should be part of the conversation from day one.
Q: Why do lifestyle changes not fix low testosterone?
A: The body needs testosterone to make lifestyle interventions work. Sleep quality, muscle synthesis, motivation, and fat metabolism all run better when the hormonal foundation is in place. Trying to fix those things without addressing clinically low testosterone is like trying to improve fuel economy in a car with a broken engine.
