  • Episode 102: Advancing Nursing Practice in Prostate Cancer Management: Clinical Evidence, Novel Agents, and Patient Care

Join UroNurse for a shared viewing of a webcast recorded at the Conversations in Primary Care: 2023 - Virtual Broadcast Live on May 20, 2023 and is being used with the permission of the presenters.

Although effective treatments are available, many patients with prostate cancer eventually experience disease progression. As novel hormonal therapies and other emerging agents are introduced earlier in the disease course, treatment decision-making is becoming increasingly personalized, requiring consideration of novel risk factors, molecular imaging, and patient preferences as well as established clinical and pathological factors. Clinicians must be aware of the recent evidence regarding new therapies and combinations and be able to support and educate patients as they weigh the benefits and risks of systemic therapy.

This enduring activity is designed to meet the educational needs of urology nurses as well as advanced practice urology nurses, nurse practitioners, and oncology nurses involved in the treatment of patients with advanced prostate cancer.

This activity is supported by unrestricted grants from Astellas and Pfizer Inc., AstraZeneca, Bayer, Janssen Biotech, Inc., administered by Janssen Scientific Affairs, LLC., Lantheus, Merck Sharp & Dohme LLC., and Novartis.

  • Episode 99: Nursing Tips for Prostate Surgery-ERAS

Enhanced Recovery After Surgery (ERAS) protocols for prostate surgery, as advocated by experts like Andrea Strong, DNP, emphasize a patient-centered approach to optimize outcomes and expedite recovery. Nurses are integral to this process, starting with preoperative education and preparation. According to Dr. Strong, it’s crucial for nurses to ensure patients understand the ERAS pathway, which includes elements like carbohydrate loading, maintaining hydration, and abstaining from smoking. Assessing and managing any comorbid conditions, optimizing nutritional status, and providing psychological support are also key preoperative tasks. By establishing a rapport and addressing any patient concerns, nurses can significantly reduce pre-surgical anxiety and improve adherence to the ERAS protocols.

Postoperatively, Dr. Strong highlights the importance of early mobilization, effective pain management, and maintaining proper hydration and nutrition. Nurses should encourage patients to begin walking and engaging in light physical activity as soon as possible to prevent complications such as deep vein thrombosis and pulmonary issues. Utilizing multimodal analgesia to minimize opioid use and its side effects is another critical aspect. Continuous monitoring for signs of complications, such as infection or urinary retention, is essential. Nurses should provide comprehensive education on self-care techniques, including catheter management if necessary. Through effective communication and patient education, nurses can empower patients to follow the ERAS guidelines, facilitating a smoother and faster recovery process and ultimately reducing the length of hospital stays.

  • Episode 98: Xtandi for Metastatic & High-Risk Nonmetastatic Castration-Sensitive Prostate Cancer

Dr. Aaron Berger discusses the crucial role of treatment intensification with Xtandi (enzalutamide) for patients with metastatic castration-sensitive prostate cancer (mCSPC) and those with nonmetastatic castration-sensitive prostate cancer (nmCSPC) experiencing biochemical recurrence at high risk for metastasis. He emphasizes that Xtandi, an androgen receptor inhibitor, offers a significant therapeutic advantage by impeding the androgen receptor signaling pathway, which is essential for the growth of prostate cancer cells. For patients with mCSPC, incorporating enzalutamide into their treatment regimen can markedly slow disease progression, reduce the risk of new metastases, and improve overall survival rates. This strategic intervention allows for better disease control and enhances the quality of life for patients undergoing treatment.

In addressing nmCSPC patients with biochemical recurrence, Dr. Berger highlights the importance of early and aggressive treatment to prevent the transition to metastatic disease. Elevated prostate-specific antigen (PSA) levels in these patients, despite castration, indicate a high likelihood of imminent metastasis. Dr. Berger advocates for the use of Xtandi in these scenarios to manage and stabilize PSA levels, thereby delaying the onset of metastasis. This proactive approach not only extends the window for effective long-term management but also aligns with a personalized treatment strategy that can significantly improve patient outcomes. Dr. Berger's expertise underscores the transformative impact of enzalutamide in managing advanced prostate cancer, providing a beacon of hope for patients through improved survival and disease management.

  • Episode 97: Vasectomy Reversal, Reclaiming Fertility

Dr. Landon Trost, a renowned expert in male fertility and microsurgery, emphasizes the transformative potential of vasectomy reversal for men seeking to reclaim their fertility. According to Dr. Trost, vasectomy reversal involves meticulously reconnecting the vas deferens, either through vasovasostomy or vasoepididymostomy, depending on the presence of blockages and the time since the original vasectomy. He highlights that advancements in microsurgical techniques have substantially increased success rates, with patency rates as high as 97% and pregnancy rates between 40-75%. Dr. Trost's expertise underscores the importance of precision and experience in performing these procedures, as they are critical factors in achieving optimal outcomes.

Dr. Trost also addresses the personal and emotional dimensions of vasectomy reversal, noting that the decision to undergo the procedure often stems from significant life changes, such as entering a new relationship or the desire to expand one’s family. He advises prospective patients to undergo a thorough consultation to understand their specific circumstances, including an evaluation of overall reproductive health and potential risks. While vasectomy reversal offers a promising path to natural conception, Dr. Trost cautions that success is not guaranteed, and additional treatments may sometimes be necessary. He emphasizes the importance of emotional support and realistic expectations, as navigating the journey toward restored fertility can be both challenging and rewarding.

  • Episode 93: Unveiling the Secrets of MRI-Guided Prostate Biopsy

Unveiling the secrets of MRI-guided prostate biopsy marks a significant leap forward in the realm of medical imaging and diagnostic precision. Leveraging the power of magnetic resonance imaging (MRI) technology, this innovative approach offers unparalleled insights into the intricate landscape of the prostate gland, allowing for more accurate detection and characterization of suspicious lesions. By integrating real-time MRI guidance with targeted biopsy techniques, clinicians can navigate with unprecedented precision, honing in on suspicious areas with pinpoint accuracy, thus minimizing unnecessary sampling and reducing the risk of false negatives. This fusion of imaging and biopsy technologies not only enhances diagnostic accuracy but also revolutionizes treatment planning, empowering healthcare providers to tailor interventions with greater precision, ultimately improving patient outcomes and quality of life.

Moreover, the unveiling of MRI-guided prostate biopsy secrets signifies a paradigm shift in prostate cancer management, ushering in an era of personalized medicine and tailored interventions. Unlike traditional blind biopsies, which often yield inconclusive results or miss clinically significant tumors, MRI-guided techniques offer a comprehensive view of the prostate gland's architecture, enabling clinicians to target suspicious regions with unparalleled precision. This transformative approach not only enhances diagnostic accuracy but also facilitates risk stratification, enabling clinicians to identify aggressive tumors early and devise tailored treatment strategies accordingly. Furthermore, by minimizing the need for repeat biopsies and reducing the risk of complications, MRI-guided prostate biopsy not only improves patient outcomes but also streamlines healthcare resource utilization, paving the way for more efficient and cost-effective prostate cancer care.
