📋 練習方式

1. 開始前先準備紙筆；旁白會先報站名與情境，聽到「Complete the notes」就開始寫

🎯 這通電話的考點

• Mechanical complications strike on days 2–7 after infarction: free wall rupture, septal rupture, papillary muscle rupture — never acute AR.

• VSR after MI: pansystolic murmur at the lower left sternal edge with a thrill; anterior MI tears the apical septum, inferior the basal.

• Papillary muscle rupture: acute MR with flash pulmonary oedema and a soft apical murmur; the posteromedial muscle has a single blood supply.

• Qp:Qs of 2.5:1 means pulmonary flow is 2.5 times systemic; above 2:1 is a large shunt. Echo is mandatory before any referral.

• IABP: inflate in diastole (coronary flow), deflate before systole (afterload); tip 2 cm below the left subclavian; no AR, no dissection.

• IABP-SHOCK II (600 patients): no 30-day mortality benefit in cardiogenic shock, so no routine use — but still a bridge for VSR and acute MR.

• Cardiogenic shock targets: noradrenaline first-line, MAP 65 mmHg or more, urine 0.5 mL/kg/h, lactate falling by over 20% within 6 hours.

• Stop ticagrelor 3–5 days before elective surgery; in shock operate now with platelets on standby. JVP up, BP down, PEA = free wall rupture.

• Vein clot: swollen, warm, bluish, pulses present. Artery clot: white, cold, pulseless. Virchow's triad: stasis, injury, hypercoagulability.

• DVT: Wells score, D-dimer (negative rules out, NPV 99%), compression ultrasound confirms; IVC filter only if anticoagulation is impossible.

• CPAP is a pneumatic splint (5–15 cmH2O); adherence = at least 4 hours a night on 70% of nights; jaw-advancement splint if intolerant.

• Heavy-vehicle licence: no driving until treated; CPAP data at 6 weeks, residual AHI under 5 = conditional licence, reviewed yearly.

• CO poisoning: PaO2 normal but co-oximetry saturation low; pulse oximetry reads falsely normal; 100% oxygen halves COHb in about 74 min.

• Stanford A (ascending aorta involved): emergency surgery, beta-blocker while theatre is prepared; Stanford B: medical, TEVAR if complicated.

OET Listening Part A 完整練習：一顆腫塊的告白。你會聽到兩位臨床人員的交班對話兩遍，邊聽邊把關鍵字寫下來（a word or a short phrase），接著逐格對答案，最後由旁白把這通電話的考點與單字唸過一遍。

📋 練習方式

1. 開始前先準備紙筆；旁白會先報站名與情境，聽到「Complete the notes」就開始寫

2. 第一遍全速聽，只抓數字、單位、藥名

3. 第二遍補完空格，注意 -teen／-ty、micrograms／milligrams、per kilogram per minute

4. 對答案時把每一格的英文說一遍（read back）

🎯 這通電話的考點

• SVC obstruction: face and both arms swell, neck veins distend, Pemberton's sign; nurse head-up and cannulate the legs, never the arms.

• Small cell lung cancer is neuroendocrine and central: limited stage fits one radiotherapy field; liver metastases make it extensive stage.

• Extensive-stage SCLC first line: carboplatin plus etoposide with a PD-L1 inhibitor, started within 48 hours when the SVC is compressed.

• Airway threatened by SVC obstruction: same-day endovascular stent; steroids only for airway compromise or lymphoma; radiotherapy for NSCLC.

• SIADH from SCLC: sodium 124, low serum osmolality, urine osmolality above 100; 1-litre fluid restriction; correct at most 8-10 mmol/L a day.

• Pancoast tumour: brachial plexus C8-T1 gives shoulder pain and hand wasting; sympathetic chain gives Horner's ptosis, miosis, anhidrosis.

• Croup: steeple sign, single-dose dexamethasone, nebulised adrenaline if stridor at rest; epiglottitis: drooling, thumb sign, no throat exam.

• TB: 2RIPE then 4RI (six months) with pyridoxine; stop if ALT over 3x ULN with symptoms or over 5x; notify public health and screen contacts.

• Tension pneumothorax is a clinical diagnosis: hypotension, trachea pushed away, full neck veins, hyperresonance; decompress before X-ray.

• Adult needle decompression: 5th intercostal space just anterior to the mid-axillary line (ATLS 10th); children 2nd ICS mid-clavicular.

• Needle buys minutes only; drain in the safe triangle (latissimus dorsi, pectoralis major, nipple line); underwater seal, no suction.

• Light's criteria, any one = exudate: protein ratio above 0.5, LDH ratio above 0.6, or pleural LDH above two-thirds of the serum upper limit.

• Drain if pleural pH below 7.2, glucose below 2.2 mmol/L, LDH above 1,000, Gram stain positive or pus; tPA + DNase (MIST2) if it fails.

• Wells above 4 = PE likely: straight to CTPA; D-dimer only rules out low probability; PERC (8 criteria) all negative skips D-dimer.

• Berlin ARDS (1 week, bilateral, not cardiac): P/F on PEEP 5 mild 200–300, moderate 100–200, severe 100 or less; 6 mL/kg predicted weight.

• CURB-65: Confusion, Urea above 7 mmol/L, RR 30 or more, BP below 90/60, age 65 or more; 0–1 home, 2 admit, 3 or more severe, consider ICU.

② 張力性氣胸是臨床診斷：RR32、SpO2 88%、HR128、BP 84/50；氣管偏對側、頸靜脈怒張、患側過度共鳴無呼吸音

③ 先針再片：第五肋間與安全三角形：成人第5肋間腋中線前方；兒童第2肋間鎖骨中線；針走肋骨上緣；針只買幾分鐘，胸管才是根本

④ Light's 三條與 pH 7.2 門檻：蛋白比大於0.5、LDH比大於0.6、LDH超上限2/3；任一達標＝滲出液；pH不在Light's裡

⑤ Wells、CTPA、PERC 與柏林定義：Wells大於4＝PE likely，直接CTPA；D-dimer只排除低機率；PERC八條全陰免驗

⑥ CURB-65 與轉診信：意識、尿素7、RR 30、BP 90/60、65歲；0–1回家、2住院、3以上重症考慮ICU
