📋 練習方式

1. 開始前先準備紙筆；旁白會先報站名與情境，聽到「Complete the notes」就開始寫

🎯 這通電話的考點

• COPD is diagnosed by a post-bronchodilator FEV1/FVC below 0.70; GOLD grades 1-4 use FEV1 % predicted cut-offs of 80, 50 and 30.

• Bronchodilator reversibility is positive only with BOTH a rise of at least 12% AND at least 200 mL in FEV1.

• Bicarbonate 34 with PaCO2 62 means chronic retention: kidneys add 3.5-4 mmol/L per 10 mmHg over days, versus 1 mmol/L acutely.

• Controlled oxygen for chronic CO2 retainers: Venturi 24-28%, target saturations 88-92%; excess oxygen raises CO2 and causes drowsiness.

• Start NIV when pH stays below 7.35 with PaCO2 above 45 mmHg after initial therapy; high CO2 is the indication, not a contraindication.

• NIV contraindications are about cooperation: coma, shock, copious secretions, vomiting or an unprotected airway; repeat the gas at 1 hour.

• OSA severity is graded by AHI: 5-14 mild, 15-29 moderate, 30 or more severe; symptomatic patients qualify from an AHI of 5.

• Obesity hypoventilation: BMI 30 or more plus awake PaCO2 above 45 mmHg with no other cause; around 90% also have OSA.

• Restriction needs a total lung capacity below 80% predicted — a low FVC alone may just be air-trapping.

• DLCO localises: low in emphysema and fibrosis, normal in asthma and neuromuscular restriction; correct it for haemoglobin.

• Peripheral favours adenocarcinoma (EGFR); central in smokers favours squamous (PTHrP) or small cell (SIADH, ACTH, Lambert-Eaton).

• Referral letter in four moves: purpose, course, current state, request — 180–200 words, Dear Dr Holt to Yours sincerely.

• Driving after CABG: private licence about 4 weeks; commercial licence about 3 months after an occupational assessment.

• Elective CABG: stop ticagrelor 3–5 days and clopidogrel 5 days; in cardiogenic shock operate now, dead myocardium cannot be restocked.

• Consent language: explain the operation, risks, benefits and alternatives, then check understanding with teach-back.

OET Listening Part A 完整練習：沉默的數字。你會聽到兩位臨床人員的交班對話兩遍，邊聽邊把關鍵字寫下來（a word or a short phrase），接著逐格對答案，最後由旁白把這通電話的考點與單字唸過一遍。

📋 練習方式

1. 開始前先準備紙筆；旁白會先報站名與情境，聽到「Complete the notes」就開始寫

2. 第一遍全速聽，只抓數字、單位、藥名

3. 第二遍補完空格，注意 -teen／-ty、micrograms／milligrams、per kilogram per minute

4. 對答案時把每一格的英文說一遍（read back）

• Grapefruit furanocoumarins block gut CYP3A4: simvastatin, lovastatin and atorvastatin levels rise; rosuvastatin and pravastatin do not.

• Triglycerides above 1,000 mg/dL with eruptive xanthomas: the danger is pancreatitis — fibrate first; bile-acid resins raise triglycerides.

• Resistant hypertension = above target on 3 drugs at adequate doses including a diuretic; unexplained hypokalaemia when young: check ARR.

• Creatinine up over 30% on an ACE inhibitor: think bilateral renal artery stenosis; BP 200/118, no organ damage = urgency, not emergency.
