Dan (@TakingOffDan) and Christy (@PilotChristy) talk about the deadliest airplane crash in the United States—the crash of American Airlines Flight 191 at Chicago O’Hare on May 25, 1979. 

The fully loaded jumbo jet experienced the separation and loss of the left engine right as the plane rotated on takeoff.  The pilots did everything correctly by the book at that time, but it wasn’t enough.  The plane banked heavily and stalled, crashing killing 271 aboard and two people on the ground.

In one of our first In The Hangar shows, Dan Millican sat down with F-4 Phantom pilot Gary Pfleiger and F-18 fighter pilot Jason Gustin.  Hear their stories about raids over the jungles, tricky carrier takeoffs and landings, and how Gary moved from fighter pilot to the CIA.

March 19, 1982, Ozzy Osbourne and his band was traveling from Knoxville, Tennessee to Orlando Florida when the bus driver decided to stop at the bus company’s owner’s house on a private airstrip.  While some of the band slept in the bus, the driver, Andy Aycok, decided to jump in a Bonanza V Tail plane for some joy flying.  On the second trip, he took guitarist Randy Rhoads and tour makeup artist Rachel Youngblood and tried to perform a low pass flyby of the tour bus.  His wing clipped the bus, causing it to hit a tree and explode into the garage, killing all three on the plane.

Dan and Christy discuss the investigation Dan made in preparation for the YouTube channel video about the crash and share some behind-the-scenes issues and maybe even disagree a little on the blame.

Colgan Air Flight 3407 was a regional flight for Continental.  On February 12, 2009, the plane departed Newark at 9:18pm EST.  The trip was mostly uneventful until the plane was instructed to intercept the approach course into Buffalo on a wintery night.  The plane was a Bombardier Q400, and repeated attempts by Air Traffic Control went unanswered.  The plane had entered a Stall and crashed into a house in Clearance Center, New York, killing the 49 people on board and one person in the house.

The NTSB report determined several key causes among which was pilot fatigue and the low number of hours of pilot experience before sitting on the flight deck of a passenger liner… the 1500 hour.

March 5, 1963, country music lost some of its top performers, headlined by Patsy Cline in a small plane crash in Tennessee. On the plane was Patsy manager and the pilot, Randy Hughes, performers Cowboy Copas and Hawkshaw Hawkins. There were no survivors.

The plane was a 1960 Piper Comanche and the Civil Aeronautics Board investigation found nothing wrong with the plane that would cause it to crash. Hughes was a low-time pilot and did not hold an Instrument Rating. Conditions at the time of the crash were reported to be IMC (instrument meteorological conditions). The Probable Cause was Hughes’s spatial disorientation as the plane flew into the terrain.

Christy Wong researched this crash in preparation for a TakingOff Accident Investigation video for the YouTube channel. But there was so much more in her investigation than she could include in the video. So Dan Millican and Christy talk about some of the steps that went into creating the video and information that she couldn’t include.
