The prevalence of chronic illness has increased worldwide requiring an interprofessional team approach in the primary care setting. This podcast showcases how a school of nursing and health care system partnered to help students in a second-degree MSN program to achieve clinical learning outcomes that complement didactic instruction in primary care.
