In this prayer of confession, David asks God to "create" in him a clean heart.  The Hebrew word, bara being translated, "create."  Only the Creator can make something of out nothing; David is essentially asking for the miracle of the new birth.  The dynamic may be different in the OT compared to NT saints, but the result is the indwelling of the Spirit of God.
