Diabetes Requirements for DOT Physical Exams

DOT physical exams are typically required for individuals who operate commercial motor vehicles (CMVs) in the United States, and the medical standards are set by the Federal Motor Carrier Safety Administration (FMCSA). Please note that regulations and requirements may change over time, so it's important to verify the latest information with the FMCSA or a certified medical examiner.

Here are some general guidelines regarding diabetes and DOT physical exams:

  1. Type 1 Diabetes: For individuals with type 1 diabetes, insulin use is typically required. To pass a DOT physical, you must demonstrate that you have stable blood sugar control and are effectively managing your condition. This may involve providing documentation of your diabetes management plan, including insulin dosage, blood sugar monitoring, and a history of regular medical check-ups.
  2. Type 2 Diabetes: For individuals with type 2 diabetes, treatment may include diet and exercise, oral medications, and/or insulin. To pass the DOT physical, you must demonstrate that your diabetes is under control, your blood sugar levels are stable, and you are compliant with your prescribed treatment plan.
  3. Medical Documentation: Individuals with diabetes will need to provide a medical history and relevant documentation, including records of blood glucose monitoring, HbA1c levels, and treatment compliance. Your healthcare provider may need to complete a Diabetes Assessment Form (MCSA-5870) as part of the evaluation.
  4. HbA1c Levels: The FMCSA typically requires that your HbA1c level be less than 10% to be eligible for a DOT medical certificate. A higher HbA1c level may disqualify you, but individual determinations may be made based on your specific situation and medical history.
  5. Vision and Neuropathy Screening: Diabetes can affect vision and peripheral nerves. A vision test and a neuropathy screening may be part of the DOT physical exam to ensure that your condition doesn't impair your ability to safely operate a commercial motor vehicle.
  6. Waivers: In some cases, individuals with diabetes may be eligible for a Diabetes Exemption Program. This program allows individuals with insulin-treated diabetes to operate CMVs under specific conditions. You'll need to apply for and receive an exemption from the FMCSA to be eligible.
