Implantable Cardioverter Defibrillator (ICD). ICDs are useful in preventing sudden death in patients with known, sustained ventricular tachycardia or fibrillation. Studies have shown ICDs to have a role in preventing cardiac arrest in high-risk patients who haven't had, but are at risk for, life-threatening ventricular arrhythmias. Cardiac ablation uses heat or cold energy to create tiny scars in your heart to block abnormal electrical signals and restore a normal heartbeat. The procedure is used to correct heart rhythm problems (arrhythmias). If a tachyarrhythmia depends on a specific pathway or ectopic site of automaticity, the site can be purposefully destroyed (ablated) to effect cure. Cardiac resynchronization therapy (CRT) involves use of a pacing system to resynchronize cardiac contraction. Such systems usually include a right atrial lead, right ventricular lead, and left ventricular lead. Leads may be placed transvenously or surgically via thoracotomy. CRT is treatment to help your heart beat with the right rhythm. It uses a pacemaker to restore the normal timing pattern of the heartbeat. The CRT pacemaker coordinates how timing of the upper heart chambers (atria) and the lower heart chambers (ventricles). It also works on the timing between the left and right sides of the heart. Atrial flutter is a rapid regular atrial rhythm due to an atrial macroreentrant circuit. Symptoms include palpitations and sometimes weakness, effort intolerance, dyspnea, and presyncope. Atrial thrombi may form and embolize. Diagnosis is by electrocardiography. Treatment involves rate control with drugs, prevention of thromboembolism with anticoagulants, and often conversion to sinus rhythm with drugs, cardioversion, or atrial flutter substrate ablation.

Bundle branch block is partial or complete interruption of impulse conduction in a bundle branch; fascicular block is similar interruption in a hemifascicle of the bundle. The 2 disorders often coexist. There are usually no symptoms, but presence of either suggests a heart disorder. Diagnosis is by electrocardiography. No specific treatment is indicated.

Torsades de pointes is a specific form of polymorphic ventricular tachycardia in patients with a long QT interval. It is characterized by rapid, irregular QRS complexes, which appear to be twisting around the electrocardiogram (ECG) baseline. This arrhythmia may cease spontaneously or degenerate into ventricular fibrillation. It causes significant hemodynamic compromise and often death. Diagnosis is by ECG. Treatment is with IV magnesium, measures to shorten the QT interval, and direct-current defibrillation when ventricular fibrillation is precipitated.

Reentrant supraventricular tachycardias (SVT) involve reentrant pathways with a component above the bifurcation of the His bundle. Patients have sudden episodes of palpitations that begin and terminate abruptly; some have dyspnea or chest discomfort. Diagnosis is clinical and by electrocardiography. Initial treatment is usually with vagotonic maneuvers. If these maneuvers are ineffective, treat with IV adenosine or nondihydropyridine calcium channel blockers for narrow QRS rhythms or for wide QRS rhythms known to be a reentrant SVT with aberrant conduction that requires atrioventricular nodal conduction. Procainamide or amiodarone are effective for other wide QRS rhythms. Synchronized cardioversion can be done for all cases in which drugs are ineffective or there is hemodynamic instability. Wolff-Parkinson-White  (WPW) (preexcitation) syndrome is the most common accessory pathway SVT, occurring in about 1 to 3/1000 people. WPW syndrome is mainly idiopathic, although it is more common among patients with hypertrophic cardiomyopathy or other forms of cardiomyopathy, transposition of the great vessels, or Epstein anomaly.
