Depression, specifically MDD is seen in 83% of those with BPD

There is significant symptoms overlap:

• Sullen presentation mimics depressive symptoms

• Suicide risk and self-harm

• Affective instability = Intense episodic dysphoria; Frequent temper outbursts, agitation, irritability mimic agitated depression

• Feelings of emptiness

• Stress-related paranoia severe dissociative symptoms mimic depression with psychotic features.

Depressive symptoms that occur in those with BPD are due to identifiable stressor (instance of rejection, perceived abandonment) and depression remits when stressor removed or relationship is restored.

Depressive symptoms may be expressed to communicate feelings, such as anger, frustration, helplessness, disappointment, etc. These tend to be due a difficulty in expressing those emotions. These states do not respond to medication and require building emotional identification and communication skills.

Treatment steps if you believe you may have depression comorbid with BPD:

1. Identify the symptoms related to BPD outside of depressive symptomatology.

a. Do they last even when trigger, emotional button, or occurrence has ended?

2. Find the underlying driving force for the BPD symptoms; this is the client’s “core content” (i.e., abandonment).

3. Determine how the core content is driving BPD symptoms? For example, if the client has abandonment issues, how do you think, feel, and behave when perceived abandonment occurs?

4. Can you recognize the connection between perceived abandonment and BPD and depressive symptom expression?

5. Learn adaptive coping strategies, such as mindfulness, relaxation, or healthy distraction, when core content is triggered.

6. Address the fear of letting go of old maladaptive patterns; “no one will love me if I’m not depressed or broken”.

7. Learn new ways to see them themselves and their world without their core content, fear, and maladaptive strategies.

Complicated grief (CG) is a prolonged, persistent, and intense grief response to a loss, usually loss of a beloved person. An individual with CG can't or doesn't adapt to the loss or integrate/accept the loss. This type of experience associated with loss is quite common for those with a PD, particularly those with BPD.

I was assisted by a grief expert, a previous professor and mentor of mine, Dr. Sherry Cormier (her website: https://www.sherrycormierauthor.com/). Dr. Cormier is a certified bereavement trauma specialist and she provides grief mentoring, speaking, and consulting in the areas of grief and loss, health and wellness, and stress management. She is the author of the book Sweet Sorrow. This book provides a narrative exploration of the grief process through examination, growth, and dedication to the continuance of the self following great personal loss.

The 8 identifiers of CG?

(Numbers 1, 6 and 7 would be much more challenging for clients with a preexisting BPD diagnosis)

1. Preoccupation with the person they lost - Clinging to the deceased person. Brooding and Rumination; Castrophizing.

2. Disengagement from other people and activities - avoidance.

3. Yearning and Craving for the deceased person

4. Less hope for the future.

5. Inability to accept the loss.

6. Intense feelings of loneliness -inability to regulate emotions. Increased feelings of guilt.

7. Disruption in self-care behaviors such as disrupted sleep, sensitivity to pain, impaired response to stress.

8. Loss of self-confidence and feelings of incompetence as identity, values, and goal-setting are impacted.

Goals of CG Treatment :

1. To restore and facilitate the natural adaptive process to grief and loss

2. To facilitate the griever's self-improvement based goals,

3. To facilitate the griever's re-engagement in the world

4. To help the griever rebuild social networks and connections

5. To help the griever manage and regulate emotions.

Treatment is an important part of the working through the CG process, just as it is working with BPD or any other PD. The hardest part can be the first step, but the hardest is also the most important.

This video discusses Self Harm and Borderline Personality Disorder to help you build your understanding so you can gain insight and control in self harm tendencies and behaviors. Self-mutilation is a complex byproduct of a wide variety of mental illnesses. It has been associated with substance abuse, eating disorders, posttraumatic stress disorder, major depressive disorder, anxiety disorders, schizophrenia, and BPD (Klonsky et al., 2003). It can be defined as the infliction of harm on the self without intent to end one’s life. Klonsky and colleagues found that 4% of the general population and between 14 and 35% of college students have engaged in some form of self-mutilation, such as cutting (70%), banging or hitting themselves (21–44%), and burning themselves (15–35%). The researchers also found no conclusive evidence to show that one gender engages in self-harm at a higher rate than another.

The first step in the sequence entails the person being devalued by an identified important other. These feelings become internalized, and the individual withdraws and begins to disconnect from his or her feelings and internal subjective state. He or she turns on the self, internalizing the identified other's view, and then engages in self-mutilation/self-sabotaging behaviors that provide relief. Relief is not the singular subjective state of lessening anxiety but takes on many forms. Various definitions of "perceived relief" are next.

In this video Shauna asks me why I decide to become a psychologist? What is it about personality that interests me and I tell her a bit about my story. We also discuss the difference between the 'surface structure' and the 'core structure' of BPD, how finding positive coping mechanisms to an emotional response is key to changing 'your role' in the emotional cycle-- although one will be tempted to go back to their normal response pattern. Who is Shauna?

Shauna was 20 years-old when she was diagnosed with Borderline Personality Disorder. Chaotic relationships, post-traumatic stress, and emotional instability were just the beginning of what became a long-term struggle of pain and suffering that demanded her attention. As a journalism student with a keen interest in psychology and multiple friends with BPD, she set out to learn everything she could about the misunderstood disorder. Through sharing stories from psychologists, friends, and the darkest depths of her own life, Shauna sheds light on the bigger picture. From treatment to relationships, cognitive distortions to stigmas, Shauna has made it her goal to show the world the truth about the disorder, and how she was able to take her life back after being diagnosed. Covert narcissism can occur in anyone, that includes your parents, friends, coworkers, bosses, or significant others.

In this video I discuss the 4 components that make up covert narcissism to help you identify it in others, or perhaps in yourself; these include self-concept, interpersonal, social, and ethical aspects.

Social components - Inferiority, Self-doubt, Feels Ashamed, Fragility

Sensitive to setbacks & criticism.

Interpersonally - Unable to depend or trust others, Envy of others’ talents, possessions, & capacity for deep relations, Lack of regard for boundaries, Disregard for others’ time.

Socially - Nags continuously, Little vocational commitment, Multiple superficial interests, Chronic boredom.

Ethics - Shifts values to gain favor, Pathological Lying, Delinquent tendencies, & Disrespect for authority.

Remember, like all disorders covert narcissism is on a continuum, mild, moderate, severe or extreme. This can be determined by the degree of express of the traits as well as the degree of impairment in socioeconomic dysfunction.
