  • Do Salicylic Acid Cleansers Actually Work? | Dermatologist QandA

In this skincare Q and A, dermatologist Dr. Dray answers questions from followers about salicylic acid cleansers, Cool Peel CO₂ laser treatments, sunscreen getting in the eyes, chalazion, sebaceous hyperplasia, post-inflammatory hyperpigmentation, collagen-boosting skincare, and eczema bumps.

Dr. Dray discusses whether salicylic acid cleansers can actually work despite being rinsed off, what to know about Cool Peel laser treatments compared with IPL, why sunscreen can be particularly uncomfortable when it gets into the eyes, and whether an eye examination could cause a long-standing chalazion to resolve.

She also explains why topical retinoids may not be particularly effective for removing sebaceous hyperplasia, what ingredients may help dark spots left behind after irritation, whether it is possible to "max out" the collagen-boosting benefits of skincare ingredients, and why some people with eczema develop small, fluid-filled bumps on their skin.

Salicylic acid is an established acne ingredient and can be effective in cleanser formulations, although leave-on products may provide a stronger or more prolonged exposure.

For sunscreen around the eyes, the American Academy of Dermatology specifically recommends sunscreen sticks as an option because they are less likely to drip into the eyes.

  • Menopause Hair Loss: Can HRT Help or Make It Worse? | Dermatologist Explains

Code DRDRAY4TH https://glnk.io/5xmkn/dr-andreasuarez48m to save $905 iRESTORE Elite — Early Labor Day Sale

Hair loss during perimenopause and menopause is incredibly common, but not all menopausal hair loss has the same cause. In this video, dermatologist Dr. Dray explains the connection between menopause, hormones, HRT, and hair loss, including why thinning hair does not automatically mean that hormone replacement therapy is the right treatment.

Dr. Dray discusses the most common causes of hair loss around menopause, including female pattern hair loss, telogen effluvium, and frontal fibrosing alopecia, as well as how declining estrogen and changes in androgen activity can affect the hair follicle.

The video also explores whether HRT can help menopausal hair loss, whether HRT can potentially contribute to hair shedding, and which evidence-based treatments may actually help with thinning hair.

Dr. Dray also discusses low-level light therapy for hair loss and shares information about the iRESTORE Elite, an FDA-cleared, non-invasive, drug-free device designed to deliver low-level red light therapy to the scalp.

    In this video:

    00:00 Introduction
00:27 Why hair loss becomes more noticeable around menopause
01:22 Low-level light therapy and iRESTORE Elite
06:05 What happens to your hormones during menopause?
07:03 What does estrogen have to do with hair?
    07:59 Female pattern hair loss
09:46 Not all menopausal hair loss is female pattern hair loss
12:18 Can HRT help with hair loss?
14:05 Can HRT actually make hair loss worse?
15:19 What can actually help menopausal hair loss?
16:29 When should you see a dermatologist?
    18:33 Final takeaway

Hair loss can have many different causes, and identifying the correct type of hair loss is an important first step toward choosing an appropriate treatment. Women experiencing sudden or significant shedding, bald patches, scalp inflammation, eyebrow loss, or progressive hairline recession should consider seeing a dermatologist for evaluation.

  • How Often Should You Really Exfoliate Your Skin?

How often should you really exfoliate your skin? In this video, dermatologist Dr. Dray explains how often to exfoliate, the difference between chemical and physical exfoliation, and how to tell when your skin is being over-exfoliated.

Exfoliation can help improve rough texture, clogged pores, keratosis pilaris, flaky skin, and the appearance of uneven skin tone. But exfoliating too frequently or combining multiple exfoliating methods can disrupt the skin barrier and lead to dryness, burning, stinging, redness, peeling, and worsening of certain skin conditions.

Dr. Dray explains why everyday habits such as shaving, using a washcloth, and cleansing can contribute to the overall amount of physical stress on the skin, even though they may not be thought of as traditional exfoliation. She also discusses the importance of regular moisturizing for supporting the skin barrier and normal shedding of dead skin cells.

The video also covers how exfoliation needs can change with aging and menopause, when skin may become drier and rougher but also more sensitive and fragile.

    Topics covered:

    0:00 Introduction
0:34 What Does Exfoliation Actually Do?
    1:38 Chemical Exfoliation
    3:10 Mechanical Exfoliation
5:02 How Often Should You Exfoliate?
    6:15 The Benefits of Exfoliation
6:51 When Exfoliation Becomes Too Much
    7:37 Don't Stack Exfoliants
    8:11 Don't Forget Moisturizer
10:33 What About Menopause and Aging?
12:12 What Should You Do If You've Over-Exfoliated?
    12:41 Final Points

Whether someone uses glycolic acid, lactic acid, salicylic acid, a facial scrub, a washcloth, or simply shaves regularly, the ideal exfoliation routine depends on the individual and how their skin responds.

Key takeaway: There is no universal answer for how often to exfoliate. Some people may benefit from exfoliating once or twice a week, while others can tolerate more frequent exfoliation. Some people may not need to exfoliate at all. Paying attention to skin sensitivity and maintaining a healthy skin barrier are essential.

  • PRP vs PRF: Which Is Better for Hair Loss and Skin Rejuvenation?

Dermatologist Dr. Dray breaks down PRP vs PRF, including how these platelet-based treatments work, what the research shows for hair loss, facial rejuvenation, wrinkles, skin texture, and under-eye concerns, and whether they are actually worth the cost.

PRP, or platelet-rich plasma, and PRF, or platelet-rich fibrin, are increasingly popular cosmetic and hair loss treatments. PRF is often marketed as a newer or more advanced version of PRP, but does the evidence actually show that PRF is better?

In this video, Dr. Dray explains the differences between platelet-rich plasma (PRP) and platelet-rich fibrin (PRF), why PRP is not a standardized treatment, and how preparation methods can affect the final product.

She also discusses the current evidence for:

    • PRP for androgenetic alopecia and hair loss
    • PRF for hair growth
    • PRP for skin rejuvenation and anti-aging
    • PRF for wrinkles, skin texture, and elasticity
    • PRP and PRF for the under-eye area
    • Whether PRF is actually better than PRP
    • What to ask before spending money on PRP or PRF treatments
    • How realistic the results are for hair loss and cosmetic rejuvenation

While there is legitimate research supporting potential benefits from PRP, particularly for androgenetic alopecia, the evidence for PRF is less mature. Research on both treatments for facial rejuvenation is also limited by small studies, differences in preparation methods, and a lack of standardized treatment protocols.

Dr. Dray also explains why a biologically plausible mechanism does not necessarily mean that a treatment is clinically superior, and why consumers should be cautious about marketing claims that PRF is automatically the "next generation" or a dramatically better alternative to PRP.

    Chapters

    0:00 Introduction
0:24 What Are PRP and PRF?
1:40 PRP Is Not One Thing
    2:27 What Is PRF?
3:28 What Does PRP Actually Do?
    4:16 PRP for Hair Loss
6:10 What About PRF for Hair Loss?
7:08 PRP and PRF for Skin Rejuvenation
8:19 What About Under the Eyes?
9:24 PRP vs PRF: Which Is Better?
10:24 Is PRP or PRF Worth the Money?
12:09 What to Ask Before Paying for PRP or PRF
    12:54 Final Takeaway

Bottom line: PRP and PRF are not pure hype, but neither should be considered a miracle treatment. PRP currently has a more established evidence base for certain types of hair loss, while PRF is promising but still requires more research. For cosmetic skin rejuvenation, both treatments may offer benefits, but the evidence is not strong enough to say that one is universally superior.

As always, Dr. Dray recommends discussing hair loss and cosmetic treatment options with a qualified medical professional who can evaluate individual circumstances and set realistic expectations.

  • Amlactin vs. Glycolic Acid: Which Is Better for Anti-Aging?

Is AmLactin or glycolic acid better for anti-aging? Dermatologist Dr. Dray breaks down the science behind these two popular alpha hydroxy acids and explains why the answer depends on the concentration, formulation, pH, and whether the product is being used at home or as an in-office chemical peel.

In this video, Dr. Dray compares 12% ammonium lactate, such as AmLactin Daily 12% Lotion, with glycolic acid specifically for anti-aging. She explains what the clinical research shows about epidermal and dermal thickness, collagen-related changes, exfoliation, skin texture, and the important differences between professional glycolic acid peels and over-the-counter glycolic acid skincare products.

She also discusses human clinical studies suggesting that 12% ammonium lactate may provide benefits beyond superficial exfoliation, including effects on both the epidermis and dermis. Dr. Dray explains why this makes 12% ammonium lactate an interesting option for an at-home anti-aging skincare routine.

    In this video:

    00:00 Introduction
00:59 What are AmLactin and glycolic acid?
03:13 Why glycolic acid is popular for chemical peels
05:51 What about 12% ammonium lactate at home?
07:50 Why does dermal thickness matter?
09:36 AmLactin and steroid-induced skin atrophy
12:04 Is AmLactin better than glycolic acid?
12:54 The problem with comparing skincare ingredients
14:40 What about glycolic acid at home?
16:33 Other anti-aging ingredients to consider
    19:59 The bottom line

AmLactin vs. Glycolic Acid for Anti-Aging

Glycolic acid has a smaller molecular size than lactic acid and can penetrate the skin efficiently. At high concentrations used in professional chemical peels, glycolic acid can produce significant exfoliation and dermal remodeling.

However, those high-strength glycolic acid peels should not be confused with typical over-the-counter glycolic acid serums and lotions. Concentration, pH, formulation, and contact time all influence how an AHA behaves in the skin.

For at-home skincare, 12% ammonium lactate has particularly interesting human clinical evidence. Research has demonstrated changes in epidermal and dermal thickness with 12% lactic acid, while another human study found that 12% ammonium lactate helped spare against corticosteroid-induced atrophy in both the epidermis and dermis.

That does not mean AmLactin is equivalent to a professional glycolic acid peel, or that it will dramatically reverse wrinkles. However, the research suggests that 12% ammonium lactate may have effects that extend beyond simple surface exfoliation.

Dr. Dray also discusses other evidence-based anti-aging ingredients, including retinoids, tretinoin, retinaldehyde, retinol, adapalene, and topical vitamin C, as well as the importance of daily sun protection for preventing photoaging.

  • Does Spironolactone Age Your Skin? The Truth About Collagen

In this skincare Q and A, dermatologist Dr. Dray answers questions from followers about photosensitivity, sunscreen, growth factors, spironolactone, oily scalp, product expiration, and benzoyl peroxide.

She discusses common causes of photosensitivity, including polymorphous light eruption (PMLE), and treatment options that may be considered depending on the diagnosis. She also explains why she does not recommend mixing different sunscreen shades, even when they come from the same brand.
