Today's episode is a quickie where we finish discussing the WHO analgesic ladder by covering step 3, which is all about opioids. We talk about the mechanism of action and main side effects of opioids. We also discuss the hydromorphone:oxycodone:morphine potency equivalencies being approximately 10:5:2, and a typical initial dose of hydromorphone being about 2-4mg PO q4h.

In this episode, we introduce the WHO analgesic ladder, which provides a basic approach to pain management that you can use on all your rotations. In part 1, we talk about step 1 (acetaminophen/NSAIDs) and step 2 ("weak"... but not really... opioids like tramadol, tapentadol, codeine/T3s, Percocet and low-dose opioids) of the ladder. Stay tuned for part 2 of this topic, where we will go over step 3 of the ladder and talk all about opioids!
