What is Cancer Staging?
Our focus today is to help you understand the cancer staging system. The stage of disease measures if the cancer has spread, and how far is has spread from where it started. Cancer staging is a way to communicate the extent of cancer in the body so that everyone involved is speaking the same language.
Cancer Staging Example: Stomach Cancer
Dr. Matzo used stomach cancer as a way to understand staging. Visit our show notes for an image and detailed explanations.
Marianne also shared about cancer sub-categories, such as 1a, 2c, etc. This further refines the definition of how far the cancer has spread.
Cancer Staging with TNM Categories
The Tumor-Node-Metastasis notation is an even more refined way to define the extent of cancer spread. TNM helps to establish the anatomic extent of the disease, and the combination of the 3 factors can define the overall tumor stage. This method allows for simplification, with cancers staged from I-IV, with stage IV being the most severe stage.

We continue our series, “Everything you always wanted to know about cancer, but didn’t have anyone to ask” with a conversation about how to make sense out of a CT scan report.
Dr. Matzo goes through an example CT for someone with metastatic prostate cancer. She discusses the different sections and highlights the "Impression" section - the sum of all the important findings. Marianne talks about how radiologists document abnormal but not concerning biology; some are things just to keep an eye on. She also shared how often things unrelated to the reason for the scan are serendipitously found and treated, like her personal example of a kidney cancer.
Charlie helped highlight how useful it is to download a copy of your reports to share between locations and to facilitate your care. We hope this helps the report feel less threatening and more a tool for you to be involved in your own treatment.

Sundowning is a behavior change in people who become agitated, uncooperative, hallucinate, pace, or have increased confusion as the sun goes down. It has been found that these behaviors do not only occur for elders with dementia, but also older adults without the disease, young people, and older dogs.

Understanding Sundowning
The behaviors that are associated with sundowning are confusion, anxiety, agitation or aggressiveness with increased motor activity (e.g. pacing, wandering), and yelling, all of which get worse as the sun is going down. So what causes sundowning? Read more below.
The Link to Circadian Rhythm
The current thinking is that these sundowning behaviors occur at sunset is because of an impairment of the circadian rhythm from damage to the sleep-promoting center in the hypothalamus and decreased production of melatonin. Listen to learn more about the cause and why it is important.
Is There a Way to Help Decrease Sundowning?
We talk about several ways to help, from adjusting light exposure through the day, to adjusting the environment and activities, to possible medications. We also talk about additional physical conditions that can contribute to sundowning that you can watch for such as urinary tract infections and dehydration.
