* O-Shot: injection of Platelet Rich Plasma (PRP) into the clitoris and G-Spot areas to aid in sensitivity, ease of orgasms, and reduce urinary leakage/urgency/frequency

Charles Runels, MD

Charles started his medical career in Primary Care. A son of Alabama, he told me of his days in the Emergency Room, his research, and battles with traditional medicine. He was a man way ahead of his time and one who wanted to carve out his own path. I can relate.

Charles found the science of blood to be fascinating so he jumped in with both feet and read everything there was about Platelet Rich Plasma. Cardiac surgeons and orthopedic surgeons were using this science to help their patients recover quickly because of its regenerative properties. Charles wondered how this could be applied to other areas of the body. What about use in the clitoris, G-Spot, penis? It is your own blood, has been used thousands of times without injury, so why not try to see if it aids in sexual function.

With himself as his own patient and with the help of adventurous others Charles set on a path of discovery. He found PRP to increase blood flow, aid healing, and improve genital sensitivity. He became the most influential non-gynecologist in the world of gynecology and sexual health. And many resented him and were jealous of his accomplishments. He was at the receiving end of lots of arrows shot at him. Mostly from the bows of gynecologists. And that is what attracted me to Charles and his story.

The above video focuses on the BioTE system of Testosterone Pellets for women. The most common form of pellet delivery in the United States. Other systems out there have similar results. I have offered this treatment for many years now with tremendous success. I have also used Testosterone Bioidentical cream for the majority of my patients with equal success. Just depends on how you want to take your hormones.

Here are my personal biases regarding Testosterone Therapy in summary:

* Testosterone is a tremendous benefit for men and women and improves the quality of life for both.

* You can get equal benefits from Pellets or Creams.

* Dosing is individualized and not one-size-fits all.

* Creams are easy to manage and can be adjusted as needed immediately without having to come into the office.

* Creams are easy to place below your belly button, inner thighs, or inner arms.

* Pellet dosing can be more challenging as you are stuck with that dose given for several months.

* Pellets can be more convenient (one and done) but you do have to come in every 3-4 months for pellet placement on your buttock area.

* Some women do not absorb creams very well and will need pellets or pills.

* Injectable testosterone damages health in the long term. Avoid them.

* I can easily manage dosing of testosterone creams with Zoom or FaceTime or Phone follow ups every 3-4 months by evaluating both the symptoms and labs of patients.

* In the long run creams are much cheaper than pellets but pellets are more convenient once placed because you do not have to remember use your creams every evening.

* You need a local and accessible Bioidentical Hormone specialist if you choose pellets because you have to come in every 3-4 months for insertions.

* It is important for the doctor to use reliable compounding pharmacies. We use Zane’s Pharmacy in Lake Forrest, California, continuously for almost 20 years. They ship to most states and are rapid in reply and service and the most price conscious for my patients. They have been the Gold Standard for decades and were instrumental in teaching the USC School of Pharmacy on how to compound bioidentical hormones. I have no financial connections with them. They are just really really good guys with the best products for my patients.

* Here is the Math for Pellets for Women: Testosterone and Estradiol Pellets are about $500+ per insertion 4x a year = $2000. You have to take Progesterone pills or creams also most of the time. Progesterone pills are cheap. Progesterone creams are also inexpensive. That comes to about $2,500 per year every year for Pellets that includes the Progesterone also.

* Here is the Math for Creams for Women: Hormone management for the first year is about $2,500 for the doctor fee plus the Pharmacy Creams at $50 a month ($600) to total $3,100 for the first year. After the first year its is $1500 a year for the doctor fee plus the pharmacy cost of $600 a year for a total of $2,100 per year every year. So in most cases creams are cheaper per year than pellets by about $400-500 per year.

* Lastly, I have Bioidentical Hormone Cream patients in all 50 states.

The O-Shot has been around for over a decade now. I have been treating patients and teaching how to do the injections since 2015. It is the use of your own blood to obtain the best Growth Factors and anti-inflammatory proteins called Cytokines and giving it back to you via an injection or device to put it deep under your skin (e.g. Predictive Permeation). The inventor, Charles Runels, MD, and his wife Alexandra Runels, MD, have changed the landscape with the introduction of the Clitoxin injection.

Many women loose sensitivity of their clitoris and G-Spot areas as they age or are traumatized by childbirth or pelvic surgeries. Menopausal changes reduce the blood flow to the superficial skin and deeper structures. Blood vessels are not as open and dilated as before and less blood carrying nourishing oxygen hits the tissues. Loss of blood flow leads to loss of collagen and elastin which leads to dry and rigid vaginas and painful dry sex. Orgasms take longer to achieve and it becomes more work to “get there.” As testosterone levels decline there can be loss of desire that goes along with loss of confidence. That is the basic core issue of aging and sexual function.

There have been treatments available to help with these conditions such as estrogen creams and pills, Carboxytherapy, and radiofrequency and laser treatments to the genital areas. PRP (Platelet Rich Plasma) has been injected into the clitoris and G-Spot areas to improve blood flow, nerve function, sensitivity, and at times arousal. The trademark name for this is “O-Shot.” It has been safe and life changing for many. Now, with the addition of botulinum toxin, there is triggering of arousal, desire, lubrication, orgasm, and sexual satisfaction. Now available to you in our offices. Clitoxin Summary of Mechanism of Action…

After Injecting botulinum neurotoxin (BoNT) into the clitoris, it is thought that (1) it is taken up by endocytosis and then migrates by axonal transport to the ganglion embedded in the wall of the vagina; (2) these ganglia then signal (through afferent autonomic nerves in the inferior hypogastric plexus) to trigger the hypothalamus (where the arousal center resides); (3) the activated hypothalamus triggers (through efferents going back to the genitalia and to the cerebral cortex) arousal, desire, and lubrication, and facilitate orgasm, combining to increase sexual satisfaction in the woman. This is in addition to the neovascularization, neurogenesis, and local vasodilation triggered by the BoNT. These effects can be amplified when the BoNT is combined with the regenerative properties of platelet-rich plasma.

After being unexpectedly recalled from his surgery fellowship at Yale to serve in Air Force hospitals during the first Gulf War, Dr. Red Alinsod discovered his passion for cosmetic gynecology.

Finally finished! Only took me 20 years to get all this content together and compacted into 30 minutes. My life’s work. My treatise on labiaplasty surgery.
