  • Randomized trial of liposomal bupivacaine on ERAS program with Larissa Meyer
    In this episode of the IJGC podcast, Editor-in-Chief Dr. Pedro Ramirez, is joined by Dr. Larissa Meyer to discuss randomized trial of liposomal bupivacaine on ERAS program. Dr. Meyer is an associate professor in the Department of Gynecologic Oncology and Reproductive Medicine with a joint appointment in the Department of Health Services Research at the University of Texas MD Anderson Cancer Center. Her research focuses on patient reported outcomes, with a focus on improving quality of life and shared medical decision making.
    Highlights:
    • To minimize bias, all surgeons on the study were all trained in injection techniques prior to enrolling patients and randomization was stratified by surgeon.
    • The addition of liposomal bupivacaine to 0.25% bupivacaine for local wound infiltration did not improve the proportion of patients who were opioid free after laparotomy.
    • The addition of liposomal bupivacaine to 0.25% bupivacaine for local wound infiltration did not improve key secondary endpoints such as patient reported pain or amount of opioids taken postoperatively.

  • Tumor Staging of Endocervical Carcinoma with Esther Oliva
    In this episode of the IJGC podcast, Editor-in-Chief Dr. Pedro Ramirez, is joined by Dr. Esther Olvia to discuss tumor staging of endocervical carcinoma. Dr. Oliva is a Pathologist at Massachusetts General Hospital and Professor at Harvard Medical School and have been the recent past president of the International Society of Gynecologic Pathologists. She was born in Barcelona and trained under three giants of pathology, Dr. Robert E. Scully, Dr. Robert H. Young, and Dr. Jaime Prat.
    Highlights
    - Create awareness of difficulties encountered by pathologists when reporting findings on different cervical specimens for endocervical adenocarcinoma
    - Highlight efforts to reach consensus among pathologists to standardize reporting for cervical specimens with adenocarcinoma
    - Highlight the importance of synchronization between FIGO, TNM, and AJCC
    - Clarify important issues in staging of endocervical adenocarcinoma including but not limited to measuring grossly visible tumor, exophytic tumors, and circumferential tumors, approach when dealing with multiple specimens or multiple tumors, and reporting tumor at margins

    - Highlight areas that need more studies in order to provide useful recommendations, including ovarian involvement by endocervical adenocarcinoma, quantification of lymphovascular invasion, or evaluation of isolated tumor Cells among others.

  • Impact of TIVA on postoperative opioid consumption with Javier Lasala
    In this episode of the IJGC podcast, Editor-in-Chief Dr. Pedro Ramirez is joined by Dr. Javier Lasala to discuss the impact of TIVA on postoperative opioid consumption. Dr. Lasala is an Associate Professor of Anesthesiology at The University of Texas MD Anderson Cancer Center in Houston, TX. He is currently the Vice Chair of Clinical Affairs in his department and has been involved with Enhanced Recovery after Surgery pathways for the past 7 years. In addition he serves as the anesthesiology lead for the gynecologic oncology program and co-lead for the thoracic surgical program.
    Highlights:
    (1) Total intravenous anesthesia (TIVA) technique reduces the amount of post-operative opioid consumption in a well-established ERAS program.
    (2) Multimodal analgesia in combination with high compliance to an ERAS program will decrease patient opioid utilization.
    (3) Pain scores were unchanged with the decreased use of opioids in the post-operative course.

    (4) Future analysis will look at the anesthesia technique and opioid utilization and the effect on patient reported outcomes.

Dr. Obermair is a gynaecological oncologist in Brisbane, Australia promoting surgery with proven better patient outcomes.

  • Recurrent Ovarian Cancer: SOC-1 Trial with Rongyu Zang and Tingyan Shi
    In this episode of the IJGC podcast, Editor-in-Chief, Dr. Pedro Ramirez, is joined by Dr. Rongyu Zang and Tingyan Shi to discuss recurrent ovarian cancer and the SOC-1 Trial.
    Professor Zang is the Director of the Department of Gynecologic Oncology at Zhongshan Hospital, Fudan University in Shanghai, China. He has been specializing in gynecologic oncology, especially in clinical and translational research of advanced and recurrent ovarian cancer for the last two decades. As the founder of Shanghai Gynecologic Oncology Group (SGOG), he has been closely associated with the Gynecologic Cancer InterGroup since 2009. Professor Zang initiated several national and international multi-center randomised clinical trials, including the dose-dense EPIC trial and the SUNNY trial for advanced ovarian cancer, as well as the secondary cytoreduction trial for recurrent ovarian cancer, and so on.
    Dr. Shi is an Associate Professor of Oncology in the Department of Gynecologic Oncology at Zhongshan Hospital, Fudan University in Shanghai, China. She has focused on clinical and translational research in gynecologic oncology for over ten years. Dr. Shi has been trained in the Young Investigator Program of Gynecologic Cancer InterGroup, and participated in several international and national multi-center randomized clinical trials for ovarian cancer.
    Highlights
    + SOC-1 selected the candidates for secondary cytoreductive surgery with iMODEL score of 4.7 or less even after the correction by PET-CT imaging.
    + Secondary cytoreductive surgery favorably impacts platinum-sensitive relapsed ovarian cancer, but incomplete resection leads to the worst survival compare to no-surgery.
    + Accumulating treatment-free survival, as an important secondary endpoint, was added to explore the long-term survival benefit affected by a high proportion of cross-over from the no surgery froup to the surgery group when overall survival data are mature.

    + In both primary and recurrent settings of ovarian cancer, randomized trials are needed to confirm the survival benefit of complete surgery followed by targeting maintenance therapy.
