Pamela Fadem addresses the rational but controversial notion that with continuing national to global inequities in health care, people who are underserved must provide their own medical solutions in situations where outside help is not forthcoming. The Hesperian Foundation's work to provide common access to these medical solutions is discussed, both on account of the service it provides globally, and the ensuing conflicts it presents for modern healthcare outreach systems.
