Not More Meds, But the Right Meds at Clarest Health

Many people are dealing with a medication mess. They get prescriptions from different specialties (neurology, cardiology, etc.), who are busy and can't always check for contraindications or adverse effects. The patient then picks up drugs at different pharmacies and uses different payment methods, so that neither pharmacies nor payers know the patient fully. No one tracks the patient as they move to a hospital, rehab, etc. This fragmented approach not only leads to confusion but also increases the risk of preventable medication errors, hospital readmissions, and poor health outcomes.In this video, CEO Hammad Shah lays out the three key goals for medication management: achieving compliance (or adherence), eliminating ineffective drugs, and warning about adverse effects.
