It should be a subset of a larger plan which again should have been broken down from a goal you have for your project. 

Create a To Do list for the day on the previous day end . This to do should have activities for the day, whatever you can complete with a bit of stretching(don't make it too easy).   

Have a column for Activities completed Yes/ No(Reasons for non completion, which is the lessons learned ). Mention potential risks & how they will be tackled. 

Morning of the day conduct a stand up meeting with each trade presenting their lists as above, to promote collaboration, which is vital. . 

Learn lessons from non complete activities, include in next day's list & keep going with this cycle.

Do we create a full Project Plan & then execute or do we plan and execute in small cycles ? 

You can call this a Lean-Project Management hybrid model or Progressive Elaboration or even Rolling Wave Planning.  But what essentially happens here is that we execute in smaller, say 2 week,  repetitive cycles. This enables any changes or deviations to be incorporated in the next cycle & keep going. 

The project is planned from Milestone to Milestone as Monthly Schedules which are further broken down to weekly plans or Schedules to get better visibility.

However much we plan a project issues come up. Project issues are disruptive & puts everyone in crisis mode.  Why do issues come up? 

Ideally they should not occur when there is a Project Risks management strategy in place. As we know issues are known after they happen catching us by surprise. Risks on the other hand are mostly (maybe upto 85 to 90%) known in advance & we are prepared for them if & when they occur. 

Issues are probably those unknown unknowns which can’t be identified in advance. Admitting that issues will come up what can the project leader do to handle them so that they can be resolved as quickly & efficiently as possible ?  

An Issue Log or Register is a necessity to begin with.   This Issue Log should be maintained on a board for all to see & regularly updated.  Issues can be controlled/prevented by identifying & preparing responses to handle impediments to daily activities in a to do list 

Assumptions, constraints & risks if handled well can also prevent Issues from coming up.

First indications are your schedule slipping & your cost going into overrun. Earned Vaue Management data is useful to track these figures. Here Cost Variance & Schedule Variance on the date of assessment will indicate cost and/ or schedule slippage. Estimate to complete, ETC & Estimate at completion, EAC are also good data for assessment  

Cost variance can be due to Scope creep, or escalation in material cost from estimate. Check assumptions of output for labour material plant & revise assumptions n estimate . Redo project estimate. Redo the WBS & get buy in from stakeholders.

 For Schedule delay look at doing more parallel activities  

Persistent delay could be due to poor risks management. Rework the risks analysis with a more elaborate effort to not miss all the possible risks. Keep tracking old & adding new risks at regular intervals, maybe once a month.      Looking for better project delivery?
