Ketamine is a medication approved for use in hospitals and other medical settings to induce anesthesia, produce relaxation, and relieve pain. Because of its hallucinogenic, and dissociative effects ketamine is used recreationally, and can potentially be associated with dangerous adverse health effects, including psychological addiction. During this interview, Dr. Prager will explain strategies to manage adverse effects of ketamine and reduce patients’ risks, including less frequent risks associated with its use in combination with birth control and opioids. Strategies for managing patients’ expectations regarding opioid use and reduce abuse will also be discussed.

In the age of immunomodulatory agents, proteasome inhibitors, and other novel agents for the treatment of multiple myeloma (MM), the role of autologous hematopoietic stem cell transplantation (ASCT) has become an area of debate. Whereas most oncologists recognize a role for ASCT in the up-front treatment of MM, this view is not universally shared, particularly when it comes to treating younger patients with MM (<65 years old). Comorbidities were cited in a recent survey of perceptions and practices of oncologists and transplant specialists as the most frequent reason younger patients with a clinical indication for ASCT would not be referred to a transplant specialist. Although advanced age and comorbidities can limit eligibility for ASCT, these concerns are not absolute contraindications for ASCT. The use of plerixafor and captisol-enabled melphalan are advancing the limits of what can be achieved by ASCT in patients with MM. Vulvar vaginal atrophy (VVA) affects about half of the 64 million postmenopausal women in the U.S. VVA is the loss of estrogen stimulation on vaginal and vulvar tissue, resulting in atrophy of these tissues and these women may suffer from symptoms, which include dyspareunia - or painful sexual intercourse. With this, dyspareunia can negatively impact a woman’s sexual function and sexual relationships, in addition to her overall quality of life. This interview will focus on the diagnosis of dyspareunia and the therapies indicated for the management of this condition.

We’ll also highlight the benefits of employing a collaborative care approach to facilitate access to opioid therapy with buprenorphine in an office-based setting.

About half of the 64 million postmenopausal women in the U.S. may suffer from VVA symptoms, which include dyspareunia - or painful sexual intercourse. Dyspareunia can negatively impact a woman’s sexual function and sexual relationships, as well as her overall quality of life. Research has demonstrated that both postmenopausal women and their healthcare clinicians want to engage in discussions surrounding the impact of dyspareunia on sexual functioning and relationships – but for a variety of reasons these discussions do not take place. This interview will focus on identifying patient and clinician factors that inhibit discussion in the office setting and ways to open the dialogue surrounding dyspareunia.
