  • Valentine's Day, Love is Never Enough, The Four Fold Way
Health Care blog review and discussion, with your host A. Patrick Jonas, MD aka, Dr Synonymous, starting with a few Tweets then Cindy on Health blogs about a possible common pathway for chronic diseases.  We get cheerful greetings from Dr Fatty and WarmSocks Valentine Day messages.  AFP Community Blog by Kenny Lin, MD then reflects on the status, or lack thereof, of Family Medicine- present and future.  Dr Jonas reads from and comments on Love is Never Enough by Aaron T. Beck, MD (psychiatrist) about how couples may overcome misunderstandings.  The nine steps to "Changing your own distortions" are reviewed from Dr. Beck's book, also.  Very useful information for any couple.   Next are items from The Four Fold Way by Angeles Arrien, PhD listing the elements of the four paths:  Warrior, Healer Teacher, and Visionary.  The Healer pays "attention to what  has heart and meaning".  A good message on Valentine's Day.  A few lame-ish items from Identity and the Life Cycle by Erik H. Erikson are followed by Dr Synonymous blog drafts that are smoldering in partially written mode.  These include trends for the year 2012, Marcus Welby, MD was angry in many shows, closing down Family Medicine, and odds and ends. More on Family Medicine next week.

  • Risks in Medical Care: Understanding Benefits and Harms:
After a brief introduction of the show and disclaimer, Dr. Jonas, your host for the Dr Synonymous Show, will review 2 patient blogs in keeping with our tradition. Special guest Dan Jonas, MD, MPH, Associate director of the Center for Evidence Based Medicine at the Shep Center of the University of North Carolina will enlighten listeners about risk communication in Family Medicine, Primary Care and General Internal Medicine. How do clinical decision makers, including patients understand risks?  Do the medical literature and  the media increase or decrease the ability to understand and share information about risks?  How do clinicians communicate about benefits and harms of tests and therapies? What ethical principles such as autonomy and beneficence impact patients and physicians when considering and clarifying benefits and harms?  What biases do physicians and patients have to be aware of when considering clinical information and situations before making decisions?  What communication strategies may help patients and their physicians overcome flawed information when making clinical decisions? We'll learn and have fun with clinical decision making concepts in this show.  (Yes, Dan is my son and I'm very proud of his career and life successes)   A chat room allows listeners to comment and interact.

  • Pain and Breathe New Life into Aging Health Care System
Health Care  blog review of Warm Socks and Wren about the pain scale.  Comments by Jim, the engineer father of The Singing Pen of Dr Jen on her blog site.  Dr Jonas then presents the online version of his presentation for the Day of Breath at the Dayton International Peace Museum on January 14.  The title "Doctor and Patient DYAD- Breathe New Life into an Aging Health Care System One Relationship at a Time" describes the intention of the presentation which is outlined on the Dr Synonymous blog site (www.drsynonymous.blogspot.com).   Human centering in health care and patient-physician relationships are an essential part of the model (the Human Centered Health Home-HCHH) that is reviewed. Appreciation through heart centered breathing leads to coherence for patient and doctor, then the dyad of both together if one follows the strategies introduced in this material regularly.  The aging health care system needs a heart based boost and coherent dyads are a refreshing step in the right direction. The exercises help listeners to make progress toward feeling "whole-hearted" and healthy engaged with their family (or other primary care) physician.

  • Patient, Med Student, and Doctor Blog Review
Health Care  blog review and discussion, starting with patient blogs: Sea Spray and Medical Mojave, followed by medical student blog: Adjacent Possible Medicine.  Dr Synonymous reviews an article from Medica Economics by Arthur Lazarus, MD, MBA amounting to a confession of undue bias by Managed Care Medical Directors.  He denied procedured to thousands of patients.  This is not surprising, considering the almost random nature of strange quality initiatives by managed care companies.  Physician blogs include The Singing Pen of Dr Jen commenting about the broad array of skills she teaches and practices as a Family Physician.  Treating and Prevention of Kidney Stones in the American Family Physician is then reviewed by Dr S, who comments about his kidney stone in 2010 and 2011.  Interesting items are noted in the Dayton Daily News about 16 deaths locally from bath salts and increasing jobs in our Medicaid Managed care company Care Source.  Dr Jonas notes further from the AMA News that "learning to cook is helping doctors to give nutritional advice".  He then ends with a few comments about a draft post he's writing in Dr Synonymous Blog about harms of statins and how to assess medications for benefits versus harms.
