On an otherwise normal Friday morning — October 1, 2027 — the healthcare safety
net for 1.3 million American citizens in Puerto Rico is scheduled to
evaporate. Not a hypothetical: a mathematically predictable event on a federal
calendar. This episode unpacks the mountain of paper behind the cliff — a
Congressional Research Service report, Puerto Rico executive orders, oversight
board budget certifications and internal task force timelines — and the
13-month window to stop it.
In this episode:
The cap. Section 1108(g): while every state gets open-ended federal
matching, Puerto Rico's Medicaid runs on a hard block grant — when the money
runs out, the island covers 100% of remaining costs from a treasury that
doesn't have it, and the system seizes up link by link: insurers, hospitals,
physicians, patients.
The hard-coded 55%. The matching formula that gives the poorest states
up to an 83% federal match would give Puerto Rico the maximum — its
poverty rate exceeds every state's. Congress excluded the territories from

people in one isolated neighborhood. That statistical absurdity is real — it's
how a hidden federal formula sets Medicare Advantage funding for 676,000
American citizens in Puerto Rico. This episode follows the paper trail of how
a technical grievance became a bipartisan fix — and how fourteen Republicans
ended up leading the charge.
In this episode:
The broken thermometer. Medicare Advantage covers roughly 96% of
Puerto Rico's Medicare-eligible population (vs. about 50% on the mainland) —
yet the benchmark formula prices the island off the tiny fee-for-service
remnant that's left, primarily veterans and low utilizers.

The paradox. Puerto Rico's Medicare Advantage market delivers
near-universal participation and dominates national five-star quality
ratings — while operating on roughly 59 cents on the dollar versus the
mainland.
The unit-price myth. Physician fees are nearly identical (a mid-level
office visit pays $59.32 on the mainland vs. $57.25 in Puerto Rico) — so
what collapsed wasn't the cost of care, it was the visible fee-for-service
claims volume, down 76% as patients moved into Medicare Advantage
exactly as the government intended.
The methodology trap. How benchmarks built on suppressed spending data
mechanically guarantee suppressed budgets: a self-reinforcing loop, not a
market outcome.
The real-world chain reaction. Hospitals at a −7.9% patient-care
margin, no commercial market to cross-subsidize; hospital staff averaging
~$33,800 against a mainland average of ~$89,000; 47.3% of physicians over
age 60; extreme shortages in pediatric critical care and neurology.
