Dr. Julian Bashir has one of his toughest patients yet.

At lunch with plain, simple Garak one afternoon, the Cardassian tailor begins to experience a severe headache. It turns out that a device in Garak's head meant to help him survive torture, is now helping him survive his exile on Deep Space Nine.

Now he's addicted to it, and the device is starting to break down after nearly two years of constant use, and Garak's life hangs in the balance.
