Right Bundle Branch Block (RBBB):
Definition: RBBB occurs when there’s a delay or blockage in the electrical impulses that travel through the right bundle branch of the heart’s conduction system.
EKG Findings:
• QRS Duration: Prolonged (>120 ms).
• V1-V2 Leads (Right Precordial Leads): RSR’ pattern (rabbit ears).
• I, aVL, V5-V6 Leads (Left Lateral Leads): Broad S waves.
Clinical Significance:
• RBBB can be benign or associated with other heart conditions like pulmonary embolism, myocardial infarction, or chronic lung disease.
• It is not typically life-threatening by itself.
Left Bundle Branch Block (LBBB):
Definition: LBBB occurs when there’s a delay or blockage in the electrical impulses that travel through the left bundle branch.
EKG Findings:
• QRS Duration: Prolonged (>120 ms).
• V5-V6 Leads (Left Lateral Leads): Broad, notched (M-shaped) R waves.
• V1-V2 Leads (Right Precordial Leads): Deep S waves.
• ST Segment and T Wave: Discordant changes (ST segment and T wave deflections opposite to the QRS complex).
Clinical Significance:
• LBBB is often associated with underlying heart disease like hypertensive heart disease, coronary artery disease, cardiomyopathy, and can indicate a higher risk of adverse cardiovascular events.
• It may be life-threatening if associated with other serious heart conditions.
Characteristic Findings:
RBBB:
• QRS complex duration >120 ms.
• RSR’ pattern in V1-V2 (right precordial leads).
• Slurred S waves in leads I, aVL, V5-V6.
LBBB:
• QRS complex duration >120 ms.
• Broad, notched R waves in V5-V6.
• Deep S waves in V1-V2.
• Discordant ST-T changes.
