Sjögren syndrome is a systemic autoimmune disease best known for causing dry eyes and dry mouth, but its effects can extend well beyond the salivary and lacrimal glands. In this video, dermatologist Dr. Dray explains Sjögren syndrome from a dermatology perspective, including the skin symptoms and other dermatologic findings that may occur with this condition.
Sjögren syndrome can be associated with dry, itchy skin, photosensitive rashes, Raynaud phenomenon, purpura, cutaneous vasculitis, hair loss, and other inflammatory skin conditions. Recognizing these findings can sometimes provide an important clue that a patient may have an underlying systemic autoimmune disease.
Dr. Dray discusses how Sjögren syndrome affects the skin, what different rashes can look like, why palpable purpura and vasculitis deserve medical evaluation, and how hair loss should be evaluated rather than automatically attributed to Sjögren syndrome.
The video also explains how Sjögren syndrome is diagnosed, including anti-SSA/Ro antibodies, Schirmer testing, salivary gland testing, and, in some cases, minor salivary gland biopsy, as well as practical approaches to treating dry, itchy skin and other skin manifestations.

Can you use azelaic acid with tretinoin or another topical retinoid? Dermatologist Dr. Dray explains how these two skincare ingredients can work together for acne, rosacea, post-inflammatory hyperpigmentation, and other common skin concerns.
Azelaic acid has anti-inflammatory, antimicrobial, and pigment-modulating properties, making it useful for acne, rosacea, and dark marks left behind after breakouts. Tretinoin and other topical retinoids work differently by helping normalize skin cell turnover, prevent clogged pores, treat acne, and improve signs of photoaging.
Dr. Dray explains why combining azelaic acid and tretinoin can be particularly useful for people dealing with acne and hyperpigmentation, how to introduce both ingredients without unnecessarily irritating the skin, and whether azelaic acid really needs to be used twice a day.
The video also explores the evidence for once-daily vs. twice-daily azelaic acid for papulopustular rosacea, why once-daily treatment may be a reasonable option for some people, and how individual skin tolerance should guide your skincare routine.
Dr. Dray also discusses using azelaic acid with adapalene, tazarotene, or trifarotene, whether retinoids can be used with rosacea, and why adding more active ingredients does not necessarily mean better results.
In this video:
• Can you use azelaic acid with tretinoin?
• How azelaic acid works for acne and hyperpigmentation
• How tretinoin works for acne and photoaging
• Why azelaic acid and retinoids can complement each other

• Azelaic acid and tretinoin for anti-aging

11:59 What About Rosacea?
12:39 What About Photoaging?
13:16 The Biggest Issue: Irritation
15:53 My Bottom Line
The key takeaway is that twice-daily azelaic acid is not automatically better than once-daily treatment. Prescription azelaic acid 15% gel is labeled for twice-daily use for rosacea, but clinical research has found comparable improvement with once-daily application for papulopustular rosacea. The ideal frequency depends on the specific product, condition being treated, tolerability, and consistency.
For many people, a simple routine of azelaic acid in the morning, tretinoin at night, moisturizer, and daily sunscreen can be an effective approach. The goal is not to use the maximum number of skincare actives, but to find a routine that provides benefits without excessive irritation.

With dengue cases being reported in Florida, dermatologist Dr. Dray explains what dengue fever can look like on the skin and the important warning signs that should prompt medical attention.
Dengue is a mosquito-borne viral infection that can cause fever, headache, muscle and joint pain, pain behind the eyes, and a characteristic rash. The skin can also develop petechiae, purpura, bruising, and other bleeding manifestations. One classic description of the dengue rash is a diffuse red or pink eruption with areas of normal or pale skin, sometimes described as “islands of sparing.”
Dr. Dray explains why a rash alone cannot diagnose dengue and why skin findings can look different across different skin tones. She also discusses petechiae, how they can appear on the skin, and why new unexplained bleeding or bruising during a febrile illness deserves medical evaluation.
Most importantly, this video covers the warning signs of severe dengue, including abdominal pain, persistent vomiting, nose or gum bleeding, vomiting blood, blood in the stool, and significant lethargy or restlessness. Warning signs can become apparent 24 to 48 hours after the fever resolves, so feeling better as the fever comes down does not necessarily mean the risk has passed
The video also explains how dengue is diagnosed, why other mosquito-borne illnesses such as Zika and chikungunya can cause similar symptoms, what to do if dengue is suspected, and how to reduce mosquito exposure and prevent mosquito breeding around the home.

Dermatologist Dr. Dray reviews the REMEDY 15% Azelaic Acid Complex Advanced Clarifying Serum, a $25 skincare serum marketed as containing a 15% azelaic acid complex. But there is an important distinction: this is not a 15% azelaic acid serum.
The formula contains 10% azelaic acid plus 5% potassium azeloyl diglycinate, also known as azeloglycine. Dr. Dray breaks down what is known about these ingredients, how potassium azeloyl diglycinate compares with azelaic acid, and whether this combination offers meaningful benefits over a traditional 10% azelaic acid product.
The video also covers the evidence for azelaic acid in acne, rosacea, and hyperpigmentation, the limited clinical evidence for potassium azeloyl diglycinate, and the other ingredients in the formula, including niacinamide, beta glucan, vitamin B12, centella asiatica, licorice root, and green tea extract.
Dr. Dray also shares her personal experience using the REMEDY serum. After a couple of weeks, she developed eyelid dryness and facial itching and discontinued the product. She notes that this is her individual experience and does not mean everyone will experience irritation.
The video also compares the REMEDY serum with the Anua 10% Azelaic Acid Serum, which Dr. Dray personally uses and repurchases. The two products have a similar overall concept and price range, but the Anua serum does not contain potassium azeloyl diglycinate or vitamin B12.
Most importantly, Dr. Dray explains why “15% azelaic acid complex” does not mean 15% azelaic acid, and why consumers should not assume that 5% potassium azeloyl diglycinate is equivalent to an additional 5% azelaic acid.
She also discusses why she would like to see prescription-strength azelaic acid become available over the counter in the United States, given its versatility in dermatology and established history of use.
