  • Does Salicylic Acid Cause Purging? Dermatologist QandA

Dermatologist Dr. Dray answers more skincare questions from viewers, covering colloidal oatmeal, retinoid purging, glycolic acid, salicylic acid cleansers, minoxidil, veinous lakes, and skin tightening after weight loss.

In this skincare Q and A, Dr. Dray explains the difference between colloidal oatmeal and Avena Sativa (oat) meal extract, whether switching from tazarotene to tretinoin can cause breakouts, and what determines how strongly a 10% glycolic acid body product exfoliates.

She also discusses treatment options for mild loose or lax skin after significant weight loss, whether veinous lakes on the lips are related to sun exposure, and why salicylic acid cleansers can still exfoliate the skin even though they are rinsed off.

Several questions address the relationship between salicylic acid and minoxidil, including an important distinction between salicylic acid and aspirin. Dr. Dray also explains why "purging" is often incorrectly blamed for breakouts and irritation from skincare products.

    In this video:

    • Colloidal oatmeal vs. Avena Sativa oat extract
    • Can you use colloidal oatmeal every day?
    • Tazarotene vs. tretinoin and breakouts
    • Is retinoid purging actually a thing?
    • Using 10% glycolic acid on the body
    • What determines the strength of glycolic acid exfoliation?
    • Skin tightening after major weight loss

    • Salicylic acid vs. aspirin and minoxidil
    • Can salicylic acid interfere with topical minoxidil?
    • Can salicylic acid cause purging?
    • Salicylic acid irritation around the eyelids and hands

  • Does Your Skin Get Used to Retinol? The Truth About Skincare “Resistance”

Does your skin really get “used to” skincare ingredients? In this video, dermatologist Dr. Dray explains the difference between skin tolerance and true resistance, with a focus on topical retinoids like tretinoin, adapalene, tazarotene, and trifarotene.

Many people wonder why a skincare ingredient that once worked so well seems less effective over time, especially when acne starts coming back after years of successful retinoid use. Dr. Dray explains why this does not necessarily mean the skin has become resistant to the ingredient, and why topical retinoids can continue to be effective as long-term acne maintenance treatments.

The video covers why acne can return while using tretinoin, how changes in hormones, treatment consistency, irritation, skincare routines, and acne patterns can affect results, and why a plateau in visible improvement is not necessarily the same as treatment failure.

Dr. Dray also discusses whether people need to switch retinoids, how to recognize when irritation is being mistaken for worsening acne, and why “my skin got used to it” is often an overly simplistic explanation for changes in skincare results.

    CHAPTERS:

0:00 Does your skin get used to skincare ingredients?

2:10 What topical retinoids actually do
3:41 Why acne can come back

9:30 Can your skin get used to other skincare ingredients?
9:56 Do you ever need to switch retinoids?
    11:54 Don't confuse a plateau with resistance
12:08 What to ask when acne returns

  • Are Gel Manicures Damaging Your Nails? A Dermatologist Explains

Can gel manicures, acrylic nails, press-on nails, and traditional nail polish damage the nails or cause skin problems? Dermatologist Dr. Dray explains the most important dermatologic hazards associated with nail products, including allergic contact dermatitis, acrylate and methacrylate allergy, HEMA allergy, nail damage, onycholysis, nail discoloration, and UV exposure from gel nail lamps.

Allergic contact dermatitis is one of the most well-documented complications of nail products. It can cause itching, redness, scaling, swelling, blisters, paronychia, fingertip eczema, nail dystrophy, and separation of the nail from the nail bed. Importantly, a nail-product allergy does not always appear around the nails. Allergens can be transferred from the fingers to the eyelids, face, neck, and other areas of the skin, making nail products an often-overlooked cause of persistent dermatitis.

The video also discusses acrylates and methacrylates, including HEMA, which are important causes of allergic contact dermatitis associated with gel and artificial nails. Once someone becomes sensitized, exposure to related acrylate-containing materials in other settings may also become clinically relevant.

Repeated manicures can also contribute to nail plate thinning, onycholysis, nail dystrophy, white discoloration, yellow or orange staining, and other nail changes. These changes can sometimes resemble nail fungus or nail psoriasis.

Gel manicures also raise questions about exposure to UVA radiation from curing lamps. Current evidence regarding the long-term skin cancer risk from UV nail lamps remains limited and conflicting, but the video discusses practical ways to reduce cumulative UV exposure.

    In this video:

    0:00 Introduction

  • Does Spironolactone Lower Libido? A Dermatologist Explains

If you take spironolactone for acne, hair loss, or unwanted facial hair, you may have wondered whether it can affect your sex drive. In this video, dermatologist Dr. Dray takes a closer look at what we actually know about spironolactone, antiandrogens, and libido in women, including what the research tells us and where the evidence is still limited.

Spironolactone is a commonly used antiandrogen in dermatology, particularly for adult female acne. While decreased libido is a recognized potential side effect, studies have not given us a reliable estimate of how frequently this occurs in women. Libido is also influenced by many factors beyond testosterone and androgen signaling, including stress, sleep, medications, relationships, pain, vaginal dryness, menopause, and overall health.

We also discuss newer research that examined sexual dysfunction diagnoses among hundreds of thousands of premenopausal women with acne. Interestingly, this observational study did not find an increased rate of coded sexual dysfunction among spironolactone users. However, this does not prove that spironolactone improves libido or that it cannot cause sexual side effects. The study relied on medical coding and highlights the need for prospective research using validated measures of sexual function.

This video also covers dose-related side effects of spironolactone, potassium and medication interactions, menstrual changes, breast tenderness, pregnancy considerations, finasteride, dutasteride, and testosterone therapy in postmenopausal women with hypoactive sexual desire disorder.

Most importantly, if you notice a meaningful change in libido after starting spironolactone or another antiandrogen, it is worth discussing with your doctor. You should not have to choose between treating your skin or hair concerns and maintaining your overall quality of life.
