Phil Cowley — better known as @philsmypharmacist — is back, and a LOT has changed since the last time we talked.

Phil built a massive social media following by doing something deceptively simple: making health information useful, understandable and entertaining. But this conversation goes well beyond health hacks.

We talk about the business behind becoming a full-time creator, brand deals, affiliate marketing and the importance of protecting the trust you’ve built with your audience. Phil explains why he believes creators can burn through their “creator equity” when every post starts feeling like a sales pitch — and why sometimes he’ll recommend a product one day and show you a cheap solution from the dollar store the next.

We also get into what it’s like being recognized everywhere he goes, including a great story involving Henry Winkler, and why Phil has developed such an appreciation for people who overcome their own hesitation to walk up and tell him his content has helped them.

Phil opens up about something else he’s been very intentional about: keeping much of his family life off social media. We talk about kids growing up online, respecting their privacy and remembering that while someone may choose to become a public figure, their spouse and children didn’t necessarily make that same choice.

And then there’s the biggest change since our last conversation.

Phil talks about stepping away from his Utah pharmacy, the regulatory scrutiny he says his business faced, and ultimately having to decide which road he wanted to take: continuing down the traditional pharmacy path or betting on the enormous opportunity he’d created through social media.

He chose the latter.

It’s a fascinating conversation about health, business, family, social media, influence and what happens when something that started as a way to help people online completely changes the trajectory of your life.
