Episode Introduction
NASA and Boeing are struggling to correct mistakes which caused recurring disasters. Listen to what High Reliability Organizations need to learn to succeed.
Psychological safety and understanding how to fail intelligently are the underpinnings of successful, learning organizations. Organizations and teams that can rapidly identify “weak signals” of anomalies; learn by rapid, intelligent fast failure; and adapt and correct will thrive and prevent otherwise imminent disasters. Come learn from the master, Harvard’s Novartis Professor of Leadership and Management, Dr. Amy C. Edmondson.

Episode Introduction
The Chinese have a significant lead in hypersonic weapons over the U.S. In every war gam scenario run to date, the side with hypersonic weapons, or more hypersonic weapons wins EVERY TIME!
Lessons learned from the National AeroSpace Plane (NASP) program in the later part of the 1980's could greatly benefit the acceleration of current hypersonics program in the United States

Episode Introduction
In every wargame scenario run, the competitor that does not have hypersonic weapons loses. Dr. Mark Lewis explains how the US lost its leadership and what it needs to do to regain it!
The United States once held a commanding lead in the field of hypersonic research. NASA and the DoD had robust X-Plane programs which explored new technology development in critical key areas like propulsion, materials, aerothermodynamics, structures, thermal control, and flight dynamics. Unfortunately, our NASA and DoD programs and funding has evaporated over the past 50 years, key research labs and wind tunnels have been closed, and our student pipeline in advanced areas of research cannot sustain the advances in other countries like China. This is the most serious threat to our security as a nation in the immediate future. Dr. Mark Lewis explains how we allowed this to happen and what steps need to be taken to regain leadership in hypersonics!

Episode Introduction
Learn how NASA helped the United States lead the development of the world’s fastest computers from Dr. Olaf Storaasli, the first person to receive Cray Computers coveted GigaFLOP Performance Award.
Although a PhD in Mechanical Engineering, Olaf envisioned computers would soon revolutionize structural analysis. He led a hardware-software-systems-application R&D team to build the NASA's first parallel computer, the Finite-Element Machine (FEM). NASA’s FEM led the way & industry soon introduced a plethora of parallel computers which now dominate supercomputing. Olaf’s General-Purpose Parallel Solver evolved with considerable advances from FEM parallel processors to Cray Supercomputers to speed Space Shuttle SRB FEM analysis of a 54,000 DoF (degrees-of-freedom) model from hours to seconds, a performance exceeding 1 petaflop for the 1st time resulting in Cray awarding Olaf & his team with the GIGAFLOP Award at the Annual Supercomputimg Conference in 1989.

Episode Introduction
NASA engineers at JSC and Boeing used a crude impact analysis which was inadequate and led to the wrong decision which resulted in the Columbia accident. Dr. Ed Fasanella was a research engineer at NASA Langley who, against the wishes of Shuttle Program Managers, led a team of NASA researchers which developed an accurate physics-based understanding of the ballistic impact which caused the Columbia accident in only 4 months.

Episode Introduction
Dr. Michael Nemeth, explains why NASA could not prevent External Tank (ET) foam shedding and why O-rings were not the cause of the Challenger accident.
For NASA to fix its broken culture, it needs to return to its research DNA which was encoded at its birth from its predecessor agency, the National Advisory Committee on Aeronautics (NACA). Dr. Michael Nemeth helps describe what a "research culture" embodies and how it differs from a sound engineering culture. He will describe an environment at NASA Research Centers like Langley in the 1980's and how that environment helped nurture some of the greatest minds which led to key knowledge discovery and technical innovations. Mike will also discuss some of the key technical challenges he and his colleagues successfully solved such as the redesign of the SRB field joint and the development of the Space Shuttle superlightweight external tank.

Episode Introduction
Ever wonder what it would be like to lead the next mission following the Columbia accident? Tune in to Leading Edge Discovery Podcast and listen to STS-114 Commander Eileen Collins’ first-hand account.
Aviation pioneer and first female Shuttle Commander, Col. Eileen Collins, recounts tales from her book, “Through the Glass Ceiling to the Stars”, about her journey growing up in Elmira, NY; flying heavy aircraft in the Air Force; Test Pilot School, and her eventual command of one of the most complex and demanding Space Shuttle missions ever.
Listen to her recount her leadership lessons learned as she was challenged to navigate an intense emotionally-charged atmosphere post Columbia, where trust was lacking and conversations were often heated. Working through several near misses which could have cost the lives of the crew, Eileen succeeded in accomplishing all mission objectives and gently landed Space Shuttle Discovery on the dry lakebed at Edwards Air Force Base in California on Aug. 9, 2005.
