Use a 4 Quadrant Priority matrix to prioritize patient needs and services under PDPM. Focus on working in the Quadrant of Quality to improve outcomes and show value.

Too many redundant goals can dilute your plan of care and make it challenging to achieve functional goals. Tune in for strategies on selecting the best goals: 1. Choose less goals 2. Pick functional goals related to the reason for referral 3. Use consistent and measurable scales 4. Always include an education goal.

Unmet goals are a direct reflection of our rehab outcomes. When a discharge is planned, all goals should be met. Ask the following 3 questions when discharge planning to avoid unmet goals and negative outcomes: 1. Which goals are unmet? Focus on these areas ONLY prior to discharge 2. Does the goal need to be downgraded or discharged? If the patient will not achieve the established goal by the planned discharge date, the goal is NOT appropriate 3. Is there a patient/caregiver education and training goal? This ensures a safe and properly trained transition helping avoid RTA.
