Social determinants of health – such as affordable and accessible housing, transportation, nutritional adequacy, and social and familial supports – may impact participant health outcomes and create challenges for providers and health plans. Partnerships built on bringing together the competencies and knowledge of medical providers and community-based organizations assist with both the transitional needs of participants as well as longer-term support for success in the community.
