The organization connects patients with resources, including a food bank, and evaluates effectiveness through data collection and community feedback, particularly focusing on the needs of the Hispanic population. Jodie notes initial hesitance from staff in asking sensitive questions but emphasizes their commitment to enhancing community health outcomes and ensuring equitable access to care.
